• Board Certified/Board Eligible ABEM or AOBEM
• Successful completion of residency
• Active WA License or IMLC and DEA
• Procedure logs showing ED volume for past 2 years, CMEs for past 2 years
• COVID, Current Flue and immunization records
• BLS, ACLS, PALS
