Position Purpose:

Under the general supervision of the Clinical Director, and clinical leadership of the Registered Nurse (RN), and in collaboration with the clinical team, the LNA performs clinical nursing procedures and functions within the Arizona (AZ) LNA Scope of Practice. The incumbent supports the plan of care for each patient, as determined by the health care team, and in accordance with the goals of the organization. Works to improve patient safety and patient experience.Key Responsibilities (List of material responsibilities and essentials duties which must be completed in achieving the objectives of the position)
Licensed Nursing Assistant:
· Responsible for attending to the daily living and safety needs of an assigned group of patients.
· Assist registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services.
· Collaborates and coordinates with other departments to provide timely effective care consistent with individual’s needs, choices and preferences.
· Obtains and records vital signs, heights and weights per policy.
· Meets patient’s personal hygiene and basic needs in a timely manner.
· Ensures that the patient is comfortable, necessary supplies are available and in reach.
· Keeps patient environment safe and tidy.
· Provides patient specific care as appropriate to age of patient.
· Demonstrates effective communication skills with children, adolescents, adults and geriatric patients to include use of appropriate therapies such as play, written, and picture board.
· Reports changes in patient’s condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor.
· Applies AIDET as well as other evidence-based behaviors.
· Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.
· Follows appropriate infection control procedures.
· Performs and accurately documents the following procedures as directed by the RN / LPN:
a. Blood glucose checks and documentation
b. Discontinuation of urinary catheters
c. Discontinuation of peripheral IVs
d. Performs skin care interventions and treatments, as permitted by state regulation.
· Maintains patient and staff confidentiality.
· Assists in the orientation of new staff as requested.
· Performs other duties as assigned.
Qualifications (Statements regarding minimum educational and experience qualifications, required proficiencies with specialized knowledge, computer proficiencies, military service, required certifications, etc.)
· Must be 18 years of age or greater
Experience: Previous experience as a Nursing Assistant, OR successful completion of a Nursing Assistant certification program
Education: High School or equivalent
Licensure: Current valid Arizona LNA License
Certification: Current BLS through the American Heart Association BLS or American Red Cross
Preferences (Preferred attributes for the position which are not absolutely required in the minimum qualifications (i.e., multi-lingual, master’s degree))
Experience: LNA or CNA experience in related unit’s fieldSpecial Position Requirements (Optional section: any travel, security, risk, hazard or related special conditions which apply to the position)

Home Health/Hospice:
Requires travel, in properly insured personal vehicle, throughout the service area.

· Able to handle multiple priorities simultaneously
· Demonstrates good oral and written communication skills
· Able to assist with the movement of an adult patient weighing up to 300 lbs.
· Able to stand, bend, stoop, kneel, and reach, while working with patients for the entire shift
· Able to care for patients with infectious diseases
· Ability to sit for 3-5 hours per day
· Able to stand for 3-5 hours per day
· Ability to walk for 3-5 hours day
· Able to care for patients in a variety of settings
· Proficiently utilize office equipment including phone, computer, fax printers, copier, scanner, etc.
