JOB DESCRIPTION PARTNERS CREATING COMMUNITY

JOB TITLE: Live in Support Service Professional (SSP) STATUS: Non-exempt
REPORTS TO: Executive Director REVISED: 08.28.2023
POSITION OVERVIEW:

The Support Service Professional (SSP) is responsible for ensuring the health, safety and ongoing supervision of Residents and Contributors receiving residential and/or day service supports in their community and home.
The SSP is responsible for assisting Residents and Contributors in the fulfillment of their ISP outcomes, when applicable, while simultaneously promoting their ongoing health, safety and overall personal growth.
Essential Duties and Functions

  • Read and assist in the implementation and development of the ISP (when applicable)
  • Provide residential supports to Resident/Contributor according to the assessment and ISP (when applicable)
  • Assist Resident/Contributor in the completion of their outcomes and residential goals.
  • Implement supports developed from plans written by the Team.
  • Remain familiar with diagnoses of Resident/Contributor within the home.
  • Assist Resident/Contributor with the development of a healthy and respectful culture.
  • Assist the Resident/Contributor to effectively engage in therapeutic/healthy relationships.
  • Enter and submit work-time in TEAMS on a regular basis by/before deadline
  • Communicate with staff at shift change about the status of the Resident/Contributor in a timely matter.
  • Check and communicate via PCC email as directed.
  • Assist in menu planning and meal preparation process alongside of the Resident/Contributor.
  • Assist/supervise the Resident/Contributor when they are completing home maintenance tasks.
  • Transport Resident/Contributor to all appointments, activities and community events.
  • Provide access to recreational and leisure activities in the home and community as well as encourage participation in activities.
  • Write thorough and effective communication, progress reports and complete data sheets as needed.
  • Administer medications to Resident/Contributor as prescribed (after completing approved training).
  • Provide medical emergency procedures as needed.
  • Identify and report any residential environmental deficiencies ASAP to the Executive Director
  • Maintain a valid drivers license and safely drive in all environments.
  • Respect Resident/Contributors culture and life choices.
  • Perform crisis intervention techniques when necessary.
  • Assist/support Resident/Contributor in maintaining good hygiene (i.e. tooth brushing and hair care).
  • Identify and report incidents to Executive Director immediately; write and submit incident report the same day that the incident is identified.
  • Attend all mandatory site staff meetings.
  • Perform all other duties as assigned by your supervisor.
Housing:

Because the position involves the care of individuals and occasionally requires the Employee to be immediately available to assist with ill or injured Residents, other events at unpredictable hours, the Employee will be provided with lodging on the premises.
The Employee is responsible for keeping the residence in clean and orderly condition and the immediate surrounding areas in tidy and appropriate condition including common areas such as the kitchen and living areas. The Employee has the option to use her/his own furnishings in the Apartment. Food, household goods, electric, gas, trash, water and Wifi services are complimentary.
Compensation:

The initial compensation for this position is $350 per month. Rent and utilities are paid for by the program.
The Employees Manger or Director will periodically meet with the Employee to provide an evaluation of the Employees performance, reviewing with the Employee both strengths demonstrated and goals for development over the coming year. As part of at least one performance evaluation each year the Employees compensation and benefits will be evaluated in the context of the Employees contribution to the overall operating performance of the Employer and may be adjusted accordingly.
Qualifications:

1. Must be at least 18 years of age
2. Masters degree preferred in social work or bachelors degree in social work with 2 years experience working with adults with disabilities.
3. Completed all required training
5. Successful completion of CPR & First Aid training required
6. Subject to criminal background check
8. Successful completion of bi-annual physical with TB test

Additional Special Working Conditions:
The SSP will have direct contact with individuals that have co-occurring disorders and Problem Sexual Behaviors (PSB) and/or Sexual Offences (SO). Willingness to work with and support individuals with MH/IDD/PSB/SO is essential for this position. The DSP will be required to assist in the daily operations of each specific program as needed or necessary.

Job Type: Full-time
Salary: $300.00 - $350.00 per month
