We are currently seeking an Independent Living Skills Trainer to join our Supportive Housing Team.

Responsibilities:

The Supportive Housing Independent Living Skills Trainer will assist Harborbrook’s 20 ESSHI participants to identify and develop strategies to increase their self-management of personal and instrumental activities of daily living.

  • As requested by the Supportive Housing Service Coordinator, assesses each of Harborbrook’s ESSHI participant’s strengths and weaknesses in performing Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) using an independent living skills functional assessment.
  • Creates an ILST plan based on the assessment that describes goals and interim steps necessary to attain the goals.
  • Provides the ESSHI participants individualized coaching and training on skills such as establishing a daily routine, household care, budgeting, bill paying, using transportation resources or other skills needed to maintain living in the community independently.
  • Re-assesses each ESSHI participant on ILST program annually and updates the plan respectively.

Qualifications:


(1) The ILST must have, at a minimum, one (1) year of experience completing functionally based assessments, developing a comprehensive treatment plan and teaching individuals with disabilities to be more functionally independent; OR

(2) An individual with a Bachelor’s degree and two (2) years of similar experience; OR

(3) An individual with a High School Diploma and three (3) years of similar experience.
