Job description

Position Summary
This position is responsible for providing guidance, education, and support to emerging adults ages 16-26 in determining the course of their life and planning goals regarding 1) Education Supports 2) Employment Skills 3) Life Development Skills 4) Health and Wellness Linkages 5) Finance & Budgeting Education and 6) Housing Resources & Supports. The position includes a specialty focus on housing navigation support and is responsible for providing information, support, and resources to homeless or unstably housed individuals and families needing emergency support. This position provides coordination of the homeless and emergency services provided within the Atlanta Metro area.
Role and Responsibilities
Life Coach: This position provides case management to the residents and the primary duties include but are not limited to the following:
  • Support clients in future planning incorporating Positive Youth Development, Motivational Interviewing, Transition to the Independence Model, and Youth Voice Youth Choice
    • Build rapport with the client being mindful of past traumas; and experiences with and perceptions of social & human-service supports.
    • Promote self-esteem which includes promoting good hygiene and appropriate personal appearance among young adults and teaching and promoting self-advocacy.
    • Reinforce socially acceptable behaviors (ex: manners, eating habits, personal safety)
    • Promote healthy boundaries and relationships with a focus on the following principles: honor, integrity, respect, and safety.
    • Teach and reinforce thoughtful decision-making, and professionalism in work tasks and employment.
    • Develop and monitor individualized service plans i.e. Goal and Life Skills Development Plans
  • Coordinating Services and Collaboration
    • Make necessary referrals to meet client's health, well-being, education, mental health, and employment and future planning needs (i.e. Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, Goodwill, Georgia Vocational Rehab, employment staffing agencies, local colleges and tech schools, etc.)
    • Actively participate in Treatment Team Meetings in which clients' 1) Urgent Needs 2) Presenting Issues 3) Updates 4) Progress 5) Successes and needed supports and interventions are collaboratively discussed.
    • Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to clients.
    • Collaborate and consult with other providers and entities in the provision of best care services to the Client, with appropriate signed consent.
  • Specific services to the client
    • Be accessible to clients as needed during hours of operations and on-call hours.
    • Teach and promote the use of SMART Goals and future planning by and with clients.
    • Evaluate individual progress and communicate such to the client and other stakeholders with client consent via a signed Release of Information (ROI) form.
    • Transport residents to and from program-related activities
    • Facilitate life skill activities (ex. conflict resolution, anger management, medication management, tutoring) to improve skills needed to live independently.
    • Providing emotional support to the client in managing daily living
  • Documentation Requirements
    • Complete training & demonstrate competency in CareLogic (Electronic Medical Records system)
    • Provide client updates and case notes in CareLogic in accordance with insurance requirements.
    • Complete Incident Reports as needed prior to shift's end.
  • Performance and Conduct
    • Maintain confidentiality of client information
    • Maintain appropriate professional boundaries.
    • Follow the CHRIS 180 Code of Ethics and core values.
    • Follow the mandated reporting requirements of GA Code
    • Support and maintain an environment of emotional and physical safety for clients, staff, and other stakeholders at all times.
  • Demonstrate customer service:
    • Always promote a positive image of CHRIS 180 programs.
    • Work as a contributing and collaborative team member
    • Respond to client and peer needs in a timely fashion.
    • Answer telephone calls and emails accordingly
    • Respond to requests for information from stakeholders in a timely fashion.
  • Maintain Personal Information and Training:
    • Attend and participate in staff meetings and training courses as scheduled.
    • Maintain required certifications.
    • Review, understand, and be responsible for program policies.
    • Provide documents in a timely fashion to Human Resources
    • Report changes in status to HR as required by policy.

Housing Navigation Assessment Duties
  • Complete Training and Meeting Requirements:
    • Client Track Training (Initial -One Time)
    • DFCS Trainings (Annually) which includes 1) Civil Rights, 2) HIPPA 3) Information Security
    • Monthly Housing Assessor Training
    • Weekly Staffing Calls (2-3 Times a Week)
    • Housing Case Management
    • Timely case notations in Client Track
  • Assessing client needs
    • Client Housing Assessments (45min - 1hour 15min per client)
    • Use the designated tool (VI-SPDAT 2.0 for individuals, families, and young adults) to prioritize individuals and families for housing.
    • Utilize results of VI-SPDAT 2.0 to prioritize individuals and households with the most intensive service needs and housing barriers.
    • Follow the protocol established within the Coordinated Entry System for referrals to housing interventions.
    • Serve as the point of contact for Coordinated Entry staff and housing providers to ensure the client is connected to housing solutions.
    • Coordinate with the entire CES team

Salary: 35k-40k
Qualifications/Competencies
  • 3 -5 years of experience working with young adults with mental illness and/or substance abuse issues.
  • A bachelor's Degree in a related field such as social work, psychology, or education counseling is preferred.
  • Working knowledge of community resources (housing and emergency interventions) within the Metro Atlanta area.
  • Knowledge of homeless databases (Client Track HMIS)
  • Technical expertise that allows well-informed decisions, usage of available tools and resources, and training to execute job responsibilities.
  • Accountable for work being completed accurately and in a timely manner as well as following up as needed.
  • Ability to collaborate with other staff to ensure best practices and sound decisions.
  • Ability to communicate using appropriate tone when corresponding verbally or in writing (as well as proper usage of grammar and spelling)
  • Ability to adapt and adjust to the course and be open to new ideas.
  • Ability to manage workload by having a clear understanding of tasks/projects and setting realistic timelines and goals.

Schedule:
Life Coaches work an average of 80 hours bi-weekly as is required to be on call monthly. The work schedule requires flexibility to meet the needs of the program and availability on an emergency basis. Primary hours are between 9:00 am-6:00 pm. Due to schedule or other operations necessities, the organization may transfer an employee to another location or shift, either on a temporary or permanent basis as this position is an employee of CHRIS 180, not the program.
