Licensed Vocational Nurse (Ssu)

Duties

• Participates in orientation of new patients to Surgical Specialty Clinics and contributes to the development of and updates to their plan of care in consideration of patient status and policy changes.
• Conducts clinical reminders as appropriate for scheduled and unscheduled patients.
• Perform vitals including height and weight on all veterans with appointments.
• Notifies Attending/Resident/Case Manager of any abnormal findings.
• Use HYPERTENSION NON-PACT OUTPATIENT CLINIC GUIDELINES for any abnormal BP reading.
• Perform EKGs for emergent/urgent patients or pre-operative patients.
• Supports data driven activities to identify patients for Surgical Specialty Clinic.
• Completes clinical documentation in the electronic medical record.
• Assist in the data collection (including physical, psychosocial, emotional, spiritual, cultural, and educational) for patients about to be and currently undergoing Surgical Specialty Clinic treatments.
• The LVN/LPN assists in screening of telephone calls and alerting providers of lab results and diagnostic tests for further review.
• Notifies the clinic manager of problems and assists in problem resolution efforts as requested.
• Prepares and administers prescribed medications (oral, topical, subcutaneous,

• Accurately and consistently identifies, documents, and reports subtle changes to veteran changes in condition.
• Is skilled at encouraging and communicating with challenging psychological and physical behaviors to promote optimal functioning.
• Uses treatment modalities and implements appropriate practices and procedures with complex patients.
• Utilizes skills to influence and communicate with difficult and challenging veterans and families.
• Demonstrates the ability to work to the capacity of his/her scope of practice and communicates effectively with the veteran, families, caregivers, professionals, and other supportive staff.
• Maintain, order and refresh stock and supplies required for ALL Surgical Specialty Clinics.
• Participates in the provision of care in the Surgical Specialty Clinics at the Loma Linda VA Medical Center, and Ambulatory Care Center, as appropriate and within the LVN/LPN Scope of Practice.
• Collaborate with Pharmacy services for medication supply.
• Screen patients in clinic and via telephone calls.
• Performs Accu-checks accurately and processes information for abnormal results.
• Enters Encounter nurse clinic visits.
• Recognizes potential urgent or emergent patient care situations, and able to rapidly call a "Rapid Response Treatment (RRT)" or "Code Blue" emergency.
• Recognizes signs of potential or active disruptive behavior and quickly intervenes with appropriate therapeutic interventions, including notifying a provider to rapidly call the BHIP provider on-call via pager.
• Demonstrates ability to use "Safe Patient Handling" equipment and techniques, able to transport, turn and move patients using proper equipment.
• Recognizes and appropriately responds to breakage/malfunction of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.
• Provides Situation, Background, Assessment and Recommendation (SBAR) report to team member.
• Clean exam and procedure rooms in between and after patient usage.
• Check for expired supplies and medications.
• Provide input for annual self-assessment.

• All clinic daily opening duties:

  • Ensure rooms are open and verify clean status of rooms.
  • Accu-check controls.
  • Medication Fridge temperature check (note this is checked twice per day).
  • Code Cart check.
  • Safe Patient Handing Equipment check:
    • HoverJack
    • HoverMatt
    • Ceiling lift
  • EKG machine check.
  • Clean and Dirty Utility Room checks.
• All clinic daily closing duties
  • Clean exam rooms for use in the AM, prepare exam tables by placing in lowered position.
  • Stocking rooms with supplies.
  • Verify no patients are remaining in clinic.
  • Lock all exams rooms.
• Daily Clinic opening duties.
  • Medication preparation and stock for the days use.
  • Temperature checks for surgery suites.
  • Fill liquid nitrogen containers.
• Daily Closing Duties
  • Collect all lab specimens.
  • Verify accuracy of all specimens prior to taking to the lab.
  • Take all specimens to the pathology section of the laboratory.
  • Waste syringes in appropriate container at end of clinic.
  • Call Pharmacy to order lidocaine syringes for following day.
  • Print Appointment list for next day and store in locked cabinet for next day.
