Job Description
JOB SUMMARY: Following a physician established plan of treatment and working under the supervision of the Registered Nurse, the Licensed Vocational Nurses (LVN) instructs patients and their families in proper treatment, help individuals and groups take steps to improve or maintain health. The LPN will record symptoms and/or reactions and progress in patients, administer medications and assist in treatment in accordance with the Physician established plan of treatment.
ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Responsible for interviewing patients, obtaining vital signs, preparing patients and assisting physicians with patient exams and treatment. 2. Charting and clerical work as necessary to oversee phone prescriptions and refills to pharmacies. 3. Perform triage with patient phone calls to identify priority and secondary situations. 4. Perform injections and specific infusions under the direction of the physician and supervising Registered Nurse. 5. Responsibility for all direct patient care activities as indicated by the Registered Nurse within the guidelines of the organization’s policies and procedures. 6. Give direct patient care according to the treatment plan and individual patient needs as directed be the Physician or Registered Nurse. 7. Provide appropriate information to assist the Registered Nurse in the preparation and maintenance of an effective patient care plan. 8. Record the patient’s official record a detailed report of care given which reflects the treatment plan and specific aspects of the patient care plan. 9. Restate instructions given to the patient by the Physician and Registered Nurse and refers any additional or new need for instruction to the Registered Nurse. 10. May administer prescribe medications to patients under the specific policies and procedures. 11. Perform selected prescribe treatment and nursing measures for the patient under specific guidelines. 12. Notify the Registered Nurse of significant findings or changes in the patient’s physical or mental conditions. 13. Ask questions when in doubt for clarification by the Registered Nurse. 14. Interact effectively with other members of the medical staff.

15. Insure privacy for patient and confidentiality of condition and personal information. 16. Prepare and recording clinical notes for the clinical records.
17. Responsible for participating in quality improvement projects/activities and accountable for quality and PCMH.
18. Perform all other duties as directed either formally or informally, verbally or in writing.
SUPERVISORY RESPONSIBILITIES:
Indirectly supervises Medical Assistants in the performance of their clinical duties and responsibilities.
KNOWLEDGE, SKILLS AND ABILITIES:
Bilingual (English/Spanish) preferred.
Strong analytical and critical thinking skills; ability to pay close attention to detail.
A high standard of professionalism and professional ethics and conduct is expected in speech, manner, attitude and appearance at all times.
Possess strong interpersonal skills and ability to work well with others.
Well-developed verbal and written communication skills and the ability to work cooperatively with divergent groups
Must possess excellent customer service including verbal and written communication.
Ability to deal effectively with changing situations and stressful environment.
Ability to perform tasks related to physical activity to complete the responsibilities of the position.
EXPERIENCE AND EDUCATION:
Current California LVN License required
Current American Heart Association (AHA) BLS certification required
Minimum of two (2) years of nursing experience preferred
