SalaryWe are seeking a Licensed Vocational Nurse to join our team! You will be responsible for delivering high quality care to assigned patients. This is a registry position with Veritas Medical Network. Candidates with experience in Long-term care are preferred.
Our first priority candidates would possess the following:
- 1-3 years’ experience providing direct care to multiple patients/residents preferred; previous experience in Skilled Nursing environment desirable
- Previous Nursing experience providing direct care to multiple patients/residents preferred
Description:
Coordinates and performs needed treatments, medications, or other nursing interventions as indicated by the patient's plan of care or as ordered by the physician. Ensures all needed emergency measures are followed as per facility policy and within standards of nursing practice. Maintains the inventory of medical supplies and medications to meet patient/resident needs. Ensures that all patient/resident rights are protected.
Responsible for ensuring the continuity of care in an outpatient setting utilizing the appropriate resources within parameters of established contracts and patients’ health plan benefits. Performs care management functions on-site or telephonically, discharge planning, utilization review and progression of care monitoring, and rounding within hospitalists. High School Diploma or GED required. Graduate of an accredited LVN Program. A.A., Diploma preferred. One to two years related experience and/or training; or equivalent combination of education and experience. Current California LVN license required.

Pay: $30.00 - $35.00 per hour
