Prime Home Health is seeking skilled and compassionate Licensed Vocational Nurses working on a PRN basis to join our growing Home Health team. LVNs in the Home Health setting play a crucial role in providing high-quality healthcare services to patients in the comfort of their San Antonio area homes - we have openings in all areas of San Antonio. Your primary responsibility will be to deliver exceptional nursing care, coordinate with the interdisciplinary healthcare team, and ensure the overall well-being and comfort of our patients.
Responsibilities:
- Provide comprehensive and compassionate nursing care to patients in their homes, adhering to established protocols and care plans.
- Assess patients' medical condition, vital signs, and overall health status on an ongoing basis.
- Administer medications, injections, and treatments as prescribed by the physician or healthcare provider.
- Assist in wound care management, including dressing changes, wound assessment, and documentation.
- Monitor and document patients' progress, symptoms, and any changes in their condition accurately and in a timely manner.
- Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized care plans for each patient.
- Educate patients and their families about their medical conditions, medications, and self-care techniques.
- Provide emotional support and address the psychosocial needs of patients and their families.
- Coordinate with other healthcare providers, such as therapists, social workers, and dieticians, to ensure comprehensive care for patients.
- Ensure proper maintenance and documentation of medical equipment and supplies.
- Adhere to infection control and safety guidelines, promoting a safe environment for both patients and caregivers.
- Participate in interdisciplinary team meetings and case conferences to discuss patient progress, goals, and care plans.
- Maintain timely, accurate and up-to-date patient records and documentation in accordance with organizational policies and regulatory requirements.
- Continuously update knowledge and skills through participation in training programs, workshops, and educational opportunities.
Qualifications:
- Current and valid LVN license in Texas.
- Minimum of 1-2 years of nursing experience, preferably in a Home Health or similar setting.
- Basic computer skills and familiarity with electronic medical record (Wellsky/Kinnser a plus) and other EMR systems.
- Valid CPR BLS certification
- Knowledge of Home Healthcare regulations, practices, and procedures.
- Strong clinical assessment and critical thinking skills.
- Proficient in administering medications and treatments.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and the healthcare team.
- Ability to work independently and demonstrate sound judgment in decision-making.
- Valid driver's license and reliable transportation for home visits.
- Ability to lift and transfer patients safely, as necessary..
- CPR certification and other relevant certifications will be required.
