Licensed Vocational Nurse (Lvn) - Prn, Home Health

Prime Home Health is seeking skilled and compassionate Licensed Vocational Nurses working on a PRN basis to join our growing Home Health team. LVNs in the Home Health setting play a crucial role in providing high-quality healthcare services to patients in the comfort of their San Antonio area homes - we have openings in all areas of San Antonio. Your primary responsibility will be to deliver exceptional nursing care, coordinate with the interdisciplinary healthcare team, and ensure the overall well-being and comfort of our patients.
Responsibilities:

1. Provide comprehensive and compassionate nursing care to patients in their homes, adhering to established protocols and care plans.
2. Assess patients' medical condition, vital signs, and overall health status on an ongoing basis.
3. Administer medications, injections, and treatments as prescribed by the physician or healthcare provider.
4. Assist in wound care management, including dressing changes, wound assessment, and documentation.
5. Monitor and document patients' progress, symptoms, and any changes in their condition accurately and in a timely manner.
6. Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized care plans for each patient.
7. Educate patients and their families about their medical conditions, medications, and self-care techniques.
8. Provide emotional support and address the psychosocial needs of patients and their families.
9. Coordinate with other healthcare providers, such as therapists, social workers, and dieticians, to ensure comprehensive care for patients.
10. Ensure proper maintenance and documentation of medical equipment and supplies.
11. Adhere to infection control and safety guidelines, promoting a safe environment for both patients and caregivers.
12. Participate in interdisciplinary team meetings and case conferences to discuss patient progress, goals, and care plans.
13. Maintain timely, accurate and up-to-date patient records and documentation in accordance with organizational policies and regulatory requirements.
14. Continuously update knowledge and skills through participation in training programs, workshops, and educational opportunities.

Qualifications:

1. Current and valid LVN license in Texas.
2. Minimum of 1-2 years of nursing experience, preferably in a Home Health or similar setting.
3. Basic computer skills and familiarity with electronic medical record (Wellsky/Kinnser a plus) and other EMR systems.
4. Valid CPR BLS certification
5. Knowledge of Home Healthcare regulations, practices, and procedures.
6. Strong clinical assessment and critical thinking skills.
7. Proficient in administering medications and treatments.
8. Excellent communication and interpersonal skills to effectively interact with patients, families, and the healthcare team.
9. Ability to work independently and demonstrate sound judgment in decision-making.
10. Valid driver's license and reliable transportation for home visits.
11. Ability to lift and transfer patients safely, as necessary..
12. CPR certification and other relevant certifications will be required.
