We are looking for an energetic and professional Full-Time Clinical LVN to join our team.
Job Summary:
The Clinical License Vocational Nurse (LVN) will act as a clinical extension for providers by carrying out clinical duties for patient care and participating in our patient-centered medical home. The LVN is a key member of the health center’s care team and works cohesively with other providers, LVNs, MAs, and other staff members. The LVN is expected to perform all designated duties, including monitoring MAs, ensuring coordination of patient care services for patients across the primary care continuum through collaboration with the patient and family to achieve optimal patient outcomes. This position requires a high level of organization and coordination. Independent judgment is required to plan, prioritize, and organize a diversified workload with professionalism and discretion. The LVN will demonstrate poise and diplomacy to a diverse group of internal and external customers. Furthermore, the LVN will participate and promote patient-centered medical home (PCMH) principles, aiding in the development of office workflows for the medical home, and collaborating with physicians and care teams in developing a standard of care with preventative services and chronic disease management. The LVN will be responsible for coordinating vaccine clinics, the nurse visit schedule, and managing patient needs through the patient portal, in conjunction with the Director of Clinical Operations.

Qualifications:
- Vocational Nurse License - required.
- Knowledge of medical terminology.
- Minimum of High School Diploma and/or equivalent.
- Minimum of 1-2 years of experience in a medical office - Required.
- Current BLS or CPR certificate.
- Excellent customer service, leadership and motivational skills.
- Excellent ability to motivate, supervise staff in a diverse working environment.
- Ability to establish and maintain effective working relationships.
- Must demonstrate strong verbal and written communication skills.
- Must demonstrate strict attention to detail.
- Ability to manage competing priorities and tasks to meet necessary deadlines.

- Knowledge of Electronic Health Record System preferred; Basic computer skills.

- Bilingual (English/Spanish) required.

- Must possess unrestricted driver’s license and auto insurance. Ability to commute between organizational sites.
Responsibilities include:
- Deals discreetly with confidential information concerning PCHC affairs, personnel and any and all related patient information.
- Addresses patient complaints and reports patient complaints to Clinic Manager. As necessary reports complaints to Director of Risk and Compliance.
- Participates in trainings for, and mentors new and existing Medical Assistant (MA) personnel.
- Works closely with the Clinic Managers and the Director of Clinical Operations on process implementations and improvements.
- Explains treatment procedures, medications, health education and other provider instructions to patients.
- Provides translation services as needed.
- Ensures that MA’s keep exam rooms stocked, clean, and organized at all times.
- Ensures contaminated supplies are disposed of appropriately.
- Disinfects and sterilizes equipment per protocol and consistently adheres to infection control policies and procedures.
- Maintains detailed vaccine logs and upholds compliance with vaccine maintenance and reporting.
- Manages vaccine clinics and supervises MA vaccine administration including verification, tracking and reporting.
- Assists with vaccine excursions and audits, and maintains compliance with various vaccine programs such as VFC and VFA.
- Addresses questions in patient portal as well as medication requests and calls from pharmacy.
- Monitors the nurse visit schedule and performs nurse visit duties such as TB readings and placements, vaccinations, PT/INR checks, BP checks, and health education visits for medication administration, diet, and exercise.
- Responsible for monitoring monthly inventory of medications and supplies including dispensary medications and proper wasting of expired meds.
- Assists as needed to ensure adequate back office flow.
- Operates electrocardiogram (EKG) machine and other equipment as needed for clinical care.
- Administers intra-dermal and intramuscular injections and inhalation treatments under the direct supervision of a licensed practitioner.
- Develops and provides educational programs for patients and families such as for diabetes, hypertension, asthma and vaccine education.
- Reviews medical plan(s) with patient/family, provides test results when appropriate to patient/family.
- Collects blood and other specimens, logs and routes specimens for testing appropriately.
- Promotes a strong productive team environment. Acts as a role model and creates a team climate characterized by honesty, trust and open communication.
- Ensures that staff are following regulatory requirements. Serves as resource to staff on regulatory requirements.
- Monitors Clinical Quality Measures. Facilitates and monitors PDSA’s (Plan, Do, Study, and Act) and other quality improvement efforts, as assigned.
- Assists, participates in, and upholds standards for various programs in preparation for audits.
- Completes special projects and other duties as assigned.
- Completes all mandatory trainings and drills.
- Assists MA Leads with audits and other duties when needed.
- Serves as a back-up to the lead MA during lead MA absence.
- Responsible for managing the LVN phone for urgent calls, critical labs/imaging, provides advice to patients and other callers under supervision of a provider.
