Job Description
As a Licensed Vocational Nurse for Outreach Health, you are responsible for providing direct nursing care on assignment from a Registered Nurse (RN). This service is to be of maximum quality and in full compliance with LVN licensure regulations and agency policy and procedures as these relate to referral source standards. S/He will ensure that patient service is delivered in full compliance with the Texas Department of Aging and Disability Services (TDADS) and Health and Human Services Commission (HHSC) rules and regulations as well as agency policies and procedures that relate to TDADS and HHSC regulations and contract standards.
Essential Job Functions
- Make visits as assigned to each patient's home, follow the patient's care plan as developed by the Registered Nurse (RN), and take the necessary action(s) to ensure our patient's well-being
- Provides direct skilled nursing to patients as assigned by an RN
- Reports any changes in the client's condition to the RN supervising the case
- Documents services are provided on appropriate forms
- Communicate patient services needed to the assigned Field Supervisor
- Participates in case conferences concerning the coordinator of client care, according to licensure regulations and contract standards
- Advises Field Supervisor of any changes in a client's schedule, caregiver, or supervisory visits
- Substitutes as needed and available for Field Supervisors in the PAS program as assigned by the Supervising Nurse or Administrator. In this position recruits, interviews, trains, hires, supervises, evaluates, and terminates (if needed) caregivers needed to provide PAS services for clients in all programs

Required Skills, Education, Certification, License, and/or Experience
- Must maintain an active and current LVN license in the working state(s) with at least one (1) year of verified experience in Home Health or another nursing discipline
- Maintain a valid driver's license and own a legally registered vehicle that holds the minimum liability insurance coverage
- Must have a clean criminal background history
- Must have knowledge of nursing procedures, treatments, and techniques
- A genuine interest and ability to work with people of all socio-economic groups
- Must be detail-oriented with strong time management skills
- Must be able to solve problems and communicate well interpersonal.
- English proficiency and excellent communication skills
Preferred Skills, Education, Certification, License, and/or Experience
- Bilingual (English and Spanish)
- Experience in Home Care
Pay Range: $22-$25/hour
