Description
Job Summary
Under the direct supervision of the Clinical Operation Supervisor and QI Clinical Director will provide nursing care in accordance with the legal scope of practice and within established standards of care, policies and procedure for a LVN.
General Responsibilities

  • Performs patient care functions, within the scope of competency, considering age related needs of the patient as prescribed by health care provider(s) following established protocols, policies and procedures.
  • Provides nursing care within licensed scope of practice
  • Determines specific needs for each clinic and prepares area with appropriate supplies, equipment, etc
  • Interviews patients and/or family, reviews patients charts and progress notes to determine prior visits to the clinic, history, status of follow-up on physicians' orders and definition of problems
  • Instructs patients' and/or family as directed by Registered Nurse or Provider
  • Assists physician with procedures and treatments
  • Participates in chart review of patients who failed clinic appointments to determine urgency of follow-up care, contacts social worker when necessary and reviews with clinic staff the plan for follow-up appointments
  • Document intake into EHR (for all specialties) and function as a system superuser for UCHC's EHR
  • Follow-up on in-basket message and management tasks
  • Maintains adequate levels of medication, supplies, and teaching aids for optimal patient care
  • Assists in orientation of new personnel
  • Participates in staff meetings and ongoing clinic nursing reviews
  • Provides nursing staff coverage in clinics as assigned
  • Assists any and all internal or external consumers with any problems they may have in a pleasant and cooperative manner
  • Responsible for collecting data from "walk-in" or "phone-in" patients and follows provider orders as directed
  • Performs, observes, and records symptoms, interactions and progress of the patient and reports findings to the medical provider
  • Assists medical providers and/or carries out medical provider's orders by performing requested tests
  • Documents and communicates activities via EHR with providers and actions taken (e.g. medication, immunization records, nursing care plans, etc.)Assist medical personnel with special procedures and minor outpatient clinical procedures.
  • Assess and monitor patients' condition and notify health care provider for disposition.
  • Prepare and administer medications as directed by health care provider order following established protocols.
  • Maintain equipment and supplies.
  • Identify and arrange for equipment needing repair.
  • Apply principles of aseptic technique and infection control as directed by the Infection Control Manual.
  • Monitor patient flow and assign/direct other ancillary personnel as needed.
  • Provide medical information and education to patients, following established protocols and guidelines.
  • Document pertinent patient information, assessment and nursing procedures following established protocols.
  • Ability to skillfully administer oral, IM and subcutaneous medications.
  • Ability to perform venipuncture with skill, adhering to all OSHA PPE safety requirements.
  • Work with the Clinical Operations Supervisor to ensure that all preventative screening services provided are done so in a culturally appropriate manner.
  • Calls patient for specific treatments/lab orders and orders laboratory work as directed by Provider
  • Provides preventive health care education and insulin teaching as well a dietary control
  • Properly and effectively use relevant computer software applications and other technologies to augment support services and complete administrative duties
  • All other duties as assigned
  • Other duties may be assigned to meet business needs

Knowledge
  • Knowledge and experience working with target populations
  • HIPAA certified and knowledge of informed consent.
  • Knowledge of medical terminology.
  • Knowledge of all safety requirements when handling bio-hazardous specimens or contaminated instruments.
  • Knowledge of MSDS and emergency treatments.
  • Knowledge of referral processing and case management of clinical referrals.

Requirements
Job Qualifications
  • Current CA LVN license.
  • Minimum of two years experience as Licensed Vocational Nurse in an Internal
  • Medicine/Family medical practice a plus.
  • Experience working in a nonprofit ASO environment a plus
  • Skill and knowledge to maintain current LVN license/certificate
  • Bilingual English/Spanish highly preferred
  • Licensed Vocational Nurse Certification in good standing
  • Minimum Certifications and/or Licenses: CPR and first aid certification required; certification as a dental assistant, preferred.
  • Minimum Skills: Experience with EMR, MS Word, Excel, MS outlook.
  • High school diploma or equivalent.
  • 1-2 years of experience directly related to the duties and responsibilities specified preferred.
  • Excellent written and verbal communication skills.
  • Ability to work under pressure without extensive supervision.

Physical Requirement
  • Physical effort which may include occasional light lifting to a 25 pound limit, and some bending, stooping or squatting. Considerable walking may be involved. The ability to sit or stand for extended periods of time is required.
  • Flexibility to work in multiple locations throughout the week.
