Job description

Position Summary/Essential Duties

         Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).

         Communicates/collaborates with the work team using a multidisciplinary approach.

         Provides linkage to community resources to facilitate optimal care.

         Provides age specific care to patients/families in all age groups.

         Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.

         Maintains a current working knowledge of UH network services and promotes utilization of such services.

         Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers.

Experience & Knowledge:

Hospital/Health Agency experience preferred.

 

Special Skills & Equipment Knowledge:

  • Demonstrates ability to interact professionally with physicians, nurses and other members of the healthcare team.
  • Demonstrates ability to apply theoretical and clinical knowledge in delivery of care.
  • Multi tasks and prioritizes work.
  • Performs autonomously as a professional.
  • Excellent verbal and written communication.

 

Education/Expertise:

Master's Degree from accredited school of Counseling.

 

Required Credentials, Licensure or Certification

Licensed by the State of Ohio Counselor, Social Worker, Marriage and Family therapist Board at the LPC level. Attainment of LPCC status is encouraged with one year of eligibility.

 

Preferred Qualifications:

The Psych Emergency Access Center is the evaluation and transfer center for the psychiatric population in all of our entity emergency departments. The LPC performs direct evaluations to determine admission or discharge. Patients are seen in-person and via telemedicine. It is a very fast-paced environment as patients from all entities are seen within one hour of referral time.  The ideal candidate will be autonomous, reliable, a critical thinker, have a crisis background, have strong passion for the psychiatric population, and be a self-starter. The position is for 8 hour shifts, day/night rotation; Monday-Friday with rotating weekends and holidays.
