Job Description
Restore First Health, a growing medical practice focused on caring for patients with chronic wounds at the bedside, is looking for a motivated LPN to perform direct patient care at patient residences and perform patient care coordination in office.
The ideal person for this type of role is a good listener, outgoing and compassionate, organized and adaptable and an excellent problem solver. This vital role works with patients, families and referring providers to ensure communication, collaboration and care coordination. They must display professionalism and maintain patient confidentiality at all times.
Requirements:
- Current Licensed Practical Nurse (LPN) in the state of Georgia
- Minimum 2 years’ experience in Wound Care
- American Heart Association (AHA) approved Basic Life Support (BLS) Certification required and must maintain thought employment
- Strong communication skills and the ability to work well with a team
- Georgia Driver's license
- Clean driving record
- Strong communication skills

- Ability to lift and move patients
- Excellent teamwork and collaboration skills
- Extensive knowledge of basic health care procedures
- Knowledge in measuring and administering medication
- Demonstrates knowledge and understanding of common wound etiologies and wound standard of care
- Drives company vehicle to patient residences
- Verify all patient appointments, medical history and chart notes.
- Assist Nurse Practitioners in and out of the field
- Work with Nurse Practitioners to communicate patient-care plans, while instructing and guiding patients, providers and families on progress.
- Change dressings and bandages, ensuring patients are healing properly
- Performs ordered treatments including but not limited to vital signs, blood sugars, vascular assessments and ABIs, wound assessments, phlebotomy, medication administration, and specimen collection and handling
- Collaboratewith referring providers to ensure timely notification of care procedures and instructions.
- Work in conjunction with Nurse Practitioners on wound product orders and supplies for each patient case.
- Works to ensure patient and family needs are being met
- Responsible for critical thinking and collaborative input on decisions

Salary: $34.00-$44.00 per hour, depending on previous experience
