The 20-bed Transitional Care Unit serves primarily short-stay recuperative and rehabilitation patients—a majority are geriatric and/or stable post-op orthopedic and oncology patients. The TCU is a family- and patient-focused environment where collaboration and communication are paramount, and you should be comfortable working with wound vacs, hoyer lifts, and standard IV therapy. You will partner with the patient and his or her family, along with a diverse roster of therapists and counselors to help the patient return home with as much independence as possible. The TCU features a wide array of recreational activities, 24-hour visitation for family and friends, and an onsite PT/OT gym. OAPSA clearance is required.
What can you Expect as a Licensed Practical Nurse?
The LPN is prepared to function as a member of the health-care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place. An LPN shall obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. An LPN shall follow the written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines and the State Practice Act. Acts within the LPN Practice guidelines (except IVs) and within the framework of Relationship Based Care to create a healing environment.
An LPN shall communicate with a licensed professional nurse and the patients' health care team members to seek guidance when:
- The patients care needs exceed the Licensed Practical nursing scope of practice.
- The patients care needs surpass the LPN's knowledge, skill, or ability.
- The patient's condition deteriorates or there is a significant change in condition, the patient is not responding to therapy, the patient becomes unstable, or the patient needs immediate assistance.
The Licensed Practical Nurse is a valued step in the UPMC nursing career ladder, with opportunities for continued growth and advancement. Salary will be determined based upon education and nursing experience.

Shifts/Hours: Our LPNs work a rotating 12-hour schedule to ensure 24/7 coverage on the floor.
- Hours are subject to change based on business needs.
- Experience in health care preferred, no LPN experience required.
- Long term care/skilled nursing care experience preferred.
- LTC/SNF preferred.
Licensure, Certifications, and Clearances:
- Licensed Practical Nurse (LPN)
- LPN must maintain state licensure where they are practicing or in a state covered by a licensure compact agreement with the state where the facility is located.
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Act 34 with renewal
