We are currently seeking a dedicated and experienced Licensed Practical Nurse (LPN) to join our team for a temporary position spanning 13 weeks. The ideal candidate will have a background in geriatric long-term care and acute rehabilitation, demonstrating proficiency in providing high-quality patient care. This opportunity is based in Canton, NY 13617, and the LPN will be responsible for a mix of 12-hour shifts, including one 8-hour shift, with weekends included on a rotating basis.

Key Responsibilities:
Provide comprehensive nursing care to geriatric and rehabilitation patients.
Administer medications and treatments in accordance with physician orders and established policies.
Monitor and assess patients' conditions, documenting observations and reporting any changes to the healthcare team.
Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement individualized care plans.
Ensure the safety and well-being of patients through adherence to established protocols and procedures.
Maintain accurate and up-to-date patient records.

Qualifications:
Current LPN license in the state of [State Name].
Minimum of 1-2 years of experience in geriatric long-term care and acute rehabilitation settings.
CPR/BLS certification.
