Poplar Care Strategies is currently adding 2 nurses to the 12 hour rotation. We offer excellent benefits, PTO accrual, 401k, week-night and weekend shift differential.
Location: Loogootee, IN, 47501Skills Required:- Licensed Nurse LPN
- Skilled nursing facility experience
- Long term care experience
- Experienced in wound care
- Experienced in dementia care
- Provide nursing care to patients in a skilled nursing facility
- Assess patient needs and develop care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient progress
- Provide wound care and manage wound healing
- Assist with activities of daily living
- Communicate with physicians, families, and other healthcare professionals
- Provide care for patients with dementia
- Current LPN license in the state of Indiana
- Minimum of 1 year experience in a skilled nursing facility
- Experience in wound care and dementia care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
