Are you an experienced Licensed Practical Nurse (LPN) in the Lemoyne, PA area looking for a position with the opportunity to work from home after 6 months? You must live within 90 minutes of the Lemoyne, PA office to be considered for this position.

UPMC Centralized Call Center is looking to add a Licensed Practical Nurse to their team to complete Transition of Care calls to PHMG patients post discharge. This position will require in-person training and work from the office for the first 6 months. After that, you could have the opportunity to work from home. Past experience as an LPN in primary care/hospital care is required for the position.

HOURS: Monday-Friday. Flexible schedule with start/stop times, must start at 8:30am or after and work 8-hour days. Remote work after 6 months, but must live in Central Pennsylvania

Responsibilities:

  • Transition of Care team, no in-person patient contact
  • Make post hospital discharge calls to patients within the UPMC Primary Care Network
  • Complete medication reconciliation with each patient
  • Schedule patients with their PCP for hospital follow-up.
  • Escalate patient concerns.
  • Comply with all policies and procedures.
  • Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).
  • Comply with all policies and procedures.
  • Document all activities/interventions, patient/family responses, etc., in the medical record.
  • Maintain strict confidentiality related to medical records and other data.
  • Promote wellness by providing patient education materials, communicating physician advice/instructions.

Qualifications:

  • Graduate of an approved LPN school of nursing.
  • One-year clinical experience preferred.
  • Experience as an LPN in primary care or hospital setting is highly desired for this position.

Licensure, Certifications, and Clearances:

  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
  • Advanced Cardiac Life Support (ACLS) OR Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Licensed Practical Nurse (LPN)
  • Act 34
