- Setting: Working with Homeless population/Shelters
- Schedule: Full time, M-F 9am-5pm. Could also be 10 or 12 hour shifts if desired.
- Pay Rate: $40-45/hr depending on experience..
The LPNs are the primary medical case manager for their assigned clients, managing client health needs by conducting health and vitals checks and coordinating with nursing leadership and other team members as needed to optimize client outcomes. They are a critical partner with the rest of the team to link clients’ medical and psychosocial needs, making referrals and suggestions for medical treatment, psychiatric rehabilitation, and coordinating information with treating physicians, pharmacies and other treatment collaterals.
