What You Will Do:
You will have the ultimate responsibility to help implement customized care plans that encompass solutions to nurture the whole person in mind, body and spirit. You will be providing impeccable care to clients by assisting in scheduled activities as well as overall care management. You will treat your clients like family, going above and beyond to enhance their quality of life.
Essential Functions:
- Providing nursing care assigned by and under the direction of a registered nurse who provides onsite supervision as needed, based upon the client’s plan of care and assessed needs.
- Supplementing the nursing care needs of the client as provided by the registered nurse.
- Participating in the nursing process by sharing in planning a client’s care, implementing a client’s plan of care, evaluating the care plan, maintaining the client’s clinical record, and recording all pertinent observations.
- Providing those services assigned in accordance with agency policies.
- Preparing clinical and progress notes for the client’s clinical record.
- Assisting the registered nurse in performing specialized procedures and duties.
- Assisting the registered nurse in carrying out the plan of care.
- Assisting the client in learning appropriate self-care techniques.
- Reporting any changes in the client’s condition to the supervising registered nurse.
- Preparing for treatments observing aseptic technique as required.
- Administering client treatments and medications as prescribed.
- Abides by the agency’s Code of Conduct.
- Abides by the agency’s infection control policies, including proper handwashing techniques consistent with CDC Guidelines.
- Assesses and manages clients who have pain.
- Other duties as assigned by supervising registered nurses.
Required Skills, Education and Certifications:
- Active New York state Licensed Practical Nurse (LPN) license in good standing
- Alzheimer's or Dementia experience is a plus
- Empathy, compassion and kindness a MUST
- Ability to use sound judgement and remain calm in a variety of situations
- Must be reliable, dependable and on time
- Must have reliable means of transportation to and from work
- Basic computer skills as well as overall technical savviness
- Proof of eligibility to work in the U.S.
