Role and Responsibilities
The Licensed Practical Nurse (LPN) is an empathetic individual with strong communication and clinical skills, critical thinking, problem-solving and decision-making skills. This position provides administrative and clinical support to Behavioral Health Medical Provider (BHMP) staff and patients and works collaboratively with the multidisciplinary team to meet the needs of the patients. They may oversee a team of behavioral health medical assistants and provide support and management for the work expectations.
Essential functions include:
- Providing scope appropriate nursing services to patients including:
- Member engagement
- Data gathering
- Care plans
- Completion of prior authorizations
- Completion of FMLA or other leave paperwork
- Patient education
- Supportive counseling
- Addressing patient inquiries as appropriate and preparing requests for BHMP's
- Providing oversight and management for MA's.
- Assists with clarifications of work expectations, support to meet those expectations and management of staff not meeting expectations.
- Works with BHMP managers to help ensure appropriate coverage for clinical hours.
- Assists in coordinating patient care with medical staff by reviewing and maintaining provider schedules and managing contact operations (inbound/outbound phone calls, emails, chats).
- Ensures patient specific needs are met by arranging interpretation services as needed and coordinating referrals to other medical professionals or team members, agencies, and community services when appropriate.
- Prepares patients for virtual appointments by ensuring the patient has what is needed for their appointment. This may include verification of text/email links, basic technology support, and check-in.
- Verifies, records, and updates patient information in EHR including chief complaint, medical history, allergies, current and discontinued medications, and lab results.
- Performs preliminary mental health screenings including but not limited to GAD-7, PHQ-9, SBIRT.
- Performs all other duties as deemed necessary and appropriate by the direct supervisor.
Qualifications and Education Requirements
- Completion of an accredited nursing program with licensure by the state nursing board as an LPN as required by the state.
- Ability to meet credentialing requirements at all work sites.
- Behavioral health experience preferred.
