Licensed Practical Nurse Lead

Our clinic operates Mondays 7:45-7pm, Tuesdays 7:45-7pm, Wednesday through Friday 7:45-5pm.  A long night is assigned to all team members, and a 1/2 day throughout the week to offset the hours of the long day.

JOB SUMMARY

Provides non-clinical and clinical duties in practice under the direction of the Registered Nurse or office manager within his or her scope by acting as a resource for CMA and LPN team members.

Acts as a preceptor for new team members during onboarding.

Patient Care: Provides assessment, evaluation, and management of patients.  

  • Participates in the collection of patient data as needed. 
  • Determines nursing care needs through recognition of relationships between data gathered and a patient's health status and treatment regimen.  
  • Identifies relevant resources based on necessity and availability. 

Communication: Communicates information to others responsible for or involved in the care of the patient. 

  • Participates in dialogue with the patient/chosen support system to identify the individualized care plan. 
  • Consistently utilizes appropriate communication tools such as Situation, Background, Assessment, Recommendation, and Question (SBARQ) when communicating within the interdisciplinary team and when handing off patient care.  
  • Documents appropriate information in a timely manner. 

Teamwork: Participates as part of the interdisciplinary care team in planning individualized care.  

  • Collaborates with the registered nurse to consider ongoing modification of care plan based on patient's needs, capabilities, and condition. 

Medical Knowledge: Demonstrates core knowledge of biomedical and clinical sciences and the application of this knowledge to patient care.  

  • Identifies nursing interventions and goals for the patient and reviews goals with the registered nurse. 
  • Identifies opportunities for process improvement, evidence-based practice, and research.  
  • Engages in self-reflection and develops personal goals to enhance knowledge, obtain professional certification, or pursue academic degree. 
  • Integrates gained knowledge into practice. 

The incumbent may be asked to perform additional duties as assigned.

 

QUALIFICATIONS

Education: High School Diploma or GED required. Graduate of an accredited practical nursing program required. Associate's Degree or Bachelor's Degree in related field preferred.

Experience: Minimum of 2 years of demonstrated leadership, training, and process improvement preferred. Experience in the physician office practice setting required. Refer to the Life Support Training Policy for additional details.

Licensure: Current LPN licensure in applicable state required. BLS required.

Additional Skills/Requirements Required: Ability to successfully complete generic and department-specific skills validation and competency testing. Has the ability to interpret information to identify each patient's requirements for care relative to his/her age specific needs. Has knowledge and skills necessary to modify care according to patient(s) age.

Additional Skills/Requirements Preferred: Leadership development.

The job requires constant standing, frequently walking, and bending/stooping. Proficient communicative, auditory, and visual skills. Attention to detail and ability to write legibly. Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
