Aurora Mental Health & Recovery (AMHR) is seeking a Full-time Licensed Practical Nurse (LPN) - Home Health Coordinator. Our LPN - Home Health Coordinator coordinates home with health care services and acts as a liaison to outside agencies and providers to ensure continuity of care for patients. This position provides coordination services throughout 3 clinics in our Adult Intensive Services department; working closely with physicians, nurse practitioners, nurses, and medical assistants. The ideal candidate should be highly collaborative, work autonomously, and enjoy working with complex systems. Preferred candidates must have previous experience working in home health. $3,000 Sign on bonus Available *Not eligible for PRN candidates, internal employees, and re-hired employees within 90 days from separation.
Based on full time (40 hours/week). Prorate if less than 40 hours/week. Paid in two installments: 50% on first paycheck, 50% on paycheck following 6-month anniversary.
Salary for this role. wage is based on licensure and company equity.
- $23.77 - $35.65 hourly
Bilingual Language Differential Pay. *rate depends on language requirements for the position and candidate's fluency.
- Will receive scheduled earnings on a bi-weekly basis for all hours worked (excluding non-productive hours paid but not worked, e.g., paid time off, holiday, administrative, etc.) in an amount equivalent to 5% of their regular base rate (hourly or salaried), to a maximum amount of $3,000.00 per calendar year. *the annual maximum amount will be prorated based on full time equivalency (FTE).
Responsibilities:
- Coordinate home health care services and act as liaison to outside agencies and providers to ensure continuity of care for patients.
- Review and verify orders for accuracy to and from home health community partners.
- Support the clinical functions of the medical team (completing all medical orders and communicating vital patient care information to physician/prescriber.)
- Perform nursing interventions within scope of practice, congruent with licensing.
- Observe, identify, and monitor the condition of the patient, including changes in symptoms, and relate the same to appropriate members of the treatment team.
- Administer injections and other medications.
- Provide medical case management and other clinical interventions, as appropriate.
- Provide assistance in crisis intervention, as needed.
- Accurately and completely chart interventions, lab results, medications administered, and all activities related to patient interactions according to the organization's procedures.
- Provide information to credentialing, payroll, etc. in a timely manner.
- Attend meetings and training as needed.
- Maintain professional licensing as required by applicable state laws.
- Other related duties as assigned.
Requirements:
- Licensure: Active State of Colorado Registered or Practical Nursing License.
- Graduation from an accredited school of nursing. Licensed as a Practical Nurse in good standing with the State of Colorado is required.
- Two years home health experience is required.
- Preferred: Two years of relevant clinical experience.
- Ability to administer medications and injections.
- Effective communication and coordination skills.
- Effective organizational and leadership skills.
- Effective problem-solving skills.
- Basic computer skills and ability to learn/utilize electronic health record.
- Ability to handle basic nursing procedures.
Required Vaccinations and TB Test.
