Position Summary:
We are seeking an experienced Licensed PracticalNurse/ HIV Early Intervention Specialist who is energetic and passionate about helping and working with a diverse population. The Licensed PracticalNurse/ HIV Early Intervention Specialist will evaluate, diagnose and treat patients. The Licensed PracticalNurse/ HIV Early Intervention Specialist will work under the supervision of the Chief Medical Officer (CMO).
Responsibilities:
- Provide specialized HIV prevention education and testing to HIV positive/negative individuals, including individualized sessions and educational workshops.
- Conduct intakes and bio-psychosocial assessments for clients living with HIV or AIDS or those affected by HIV/AIDS
- Develop comprehensive, individualized care plans to ensure access to medical care
- Regular monitoring/follow-up through both phone and face-to-face visits
- Provide crisis intervention as needed
- Participate in off-site community outreach events
- Link newly diagnosed individuals to care services, including case management, partner services, medical care, social programs, and education.
- Participate in Saint James Health's monthly Quality Management meetings
- Establish a compassionate environment by providing psychological support to patients, family and friends
- Involved with direct patient physical assessment, education, and health planning
- Assists in the coordination of services with SJH Case Managers, other disciplines and patient's insurance plans
- Advocates for patient in arrangement of medically necessary services;
- Communicates with community resources to ascertain adequate assistance for patients in need;
- Refers patients to specialty services when necessary;
- Consults with collaborating physician and other members of the health care team to establish plan of care;
- Maintains continuity of care by regular patient follow-up;
- Participates in quality assurance process to assure optimal care delivery;
- Monitors patient outcome in accordance to the practice protocols;
- Perform HIV testing, including rapid testing
- Conduct pre-test and post-test counseling
- Provide monthly HIV education
- Maintain patient records, track service due dates, and identify potential beneficiaries
- Promote Pre-Exposure Prophylaxis (PrEP) education and availability
- Monitor and coordinate PrEP patients' medical care and insurance authorization
- Assist newly diagnosed patients in transferring to HIV Retention and Adherence Program
- Prepare data reports for external agencies and clinic management
- Maintain accurate clinical documentation in electronic medical records (EMR)
- Ensure patient records integrity and confidentiality
- Perform phlebotomy services
- Adhere to safety procedures and regulations
- Maintain professional certifications and licensures

Qualifications:
- Minimum 3-years of experience as Licensed Practitioner Nurse
- Licensed Practical Nursing Sate certificate required
- Highly organized with an attention to detail
- Basic CPR (BCLS) certification required
- Phlebotomy Certification required
- Bilingual/ fluent in Spanish a must
- Experience working with HIV patients a plus
- Excellent interpersonal skills with patient and co-workers
