JOB SUMMARY
Hamilton Physician Group (“HPG”) is a multi-subspecialty physician group practice and wholly owned subsidiary of Hamilton Medical Center, Inc., (“HMC”). HMC is a wholly owned subsidiary of Hamilton Healthcare System, Inc. (“HHCS”). HPG was created as a responsive, scalable and adaptable physician enterprise to meet the long-term community need for medical specialties within the Dalton, Georgia and surrounding communities that for various reasons may not be optimally provided for or sustainable by the independent medical staff.
The Clinical Lead / Wellness Nurse will plan, organize, supervise and lead the day to day operations of clinical staff in the outpatient clinic ensuring the optimal, quality patient and family centered care. This includes staff leadership in clinical processes, staffing, scheduling, training, workflow processes, quality improvement and risk reduction initiatives. The Clinical Lead / Wellness Nurse integrates care and services with an emphasis on preventative care and education.
To perform nursing services and set standards to assure professional, skilled program of care and treatment is administered in a physician office setting. The Clinical Lead / Wellness Nurse is responsible for contacting eligible participants and scheduling wellness visit. Wellness visits are to provide patients with an assessment of wellness, prevention and risk. The Clinical Lead / Wellness Nurse will work closely with the health care team to establish a care plan for the patient. In addition, the Clinical Lead / Wellness Nurse will be a resource to provide education and development of other wellness programs. Responsible for accommodating the needs of the physician by controlling patient flow, preparing patients for exams, aiding physician with routine procedures, preform health risk assessments, assist with recommendations and referrals, and discuss with participant other issues such as Advanced Directive and Living Will.
The Clinical Lead / Wellness Nurse will provide also necessary education to the patient based on their need and clinical conditions.
JOB QUALIFICATIONS
EDUCATION: Graduate of an accredited Licensed Practical Nurse Program (LPN) or Graduate of an accredited Registered Nurse (RN) program preferred
Licensure: Registered LPN license, or eligible for license, in state of Georgia. AHA Healthcare Provider BLS or ARC. Professional Rescuer BLS. Registered RN License preferred Registered LPN License; AHA Healthcare Provider BLS
EXPERIENCE: Preferred experience in a clinical or hospital setting. Previous wellness/ health coaching experience desired
JOB SKILLS: Nursing skills as defined in the Law Governing the Practice of Nursing in Georgia. Knowledge of clinical patient care, the ability to use critical thinking skills, highly organized with the ability to maintain accurate documentation and reporting, demonstrates evidence of exceptional leadership skills, positive communication, and collaboration with patients, families and other health care team members, maintains core values that are consistent with quality patient/ family centered care, and maintains a positive attitude toward other team members.
