Job Description
Type: 3+ month contract, with opportunity for extension
Schedule: 8 am to 5 pm, Monday - Friday (1 hour lunch)
Pay: $32 - $34.87/hr
Dress: Solid color scrubs
Responsibilities of the LPN:
• Case Management
• Clinical Support to Medical Assistants
• Working with Providers on high-risk patients
• Daily, Weekly, Monthly follow up of high-risk patients
• Coordinating all post discharge, ER visit plan of care
Qualifications for the LPN:
· High school diploma / GED
· Must have LPN license in Florida
· Case management experience

Pay Details: $32.00 to $34.87 per hour
