Royalty Adult & Pediatric Home Care is looking for a compassionate and motivated LPN as we work to expand our team to provide high quality services!
The client care coordinator manages and coordinates all the care components for our clients. Care components include new client and staff onboarding, scheduling, care implementation, resource and referral coordination, teaching, training and coaching. This role also get the opportunity to provide support, guidance and leadership to unlicensed caregiver staff and monitors care outcomes.
Work Schedule: Monday - Friday 8a-5p with on-call rotation
Annual Starting Base Salary: $45,760 - $52,000

ResponsibilitiesUtilizing the critical thinking skill, experience and training the LPN Care Coordinator can expect the responsibilities.
  • Work under the Supervision of the Registered Nurse Supervisor
  • Carry a caseload of assigned clients while continuing to grow caseload and client hours.
  • Provides professional nursing care by utilizing all elements of nursing process: Assessment, Planning, Implementation, Evaluation utilizing company processes and procedures within scope of practice.
  • Assesses and evaluates client's status by:
    • Reviewing Plan of Care
    • Regularly re-evaluating client needs
    • Requesting updates to the Plan of Care as necessary
  • Conduct Home Visits to:
    • Enroll new clients into program and establish care relationship
    • Serve as key point of contact for all established client needs
    • Routine check ins to monitor care progress and screen for needs
  • Select appropriate caregivers and Schedule Client services
  • Monitor client scheduling, care delivery, and field documentation
  • Assist with caregiver interviewing and orientation
  • Meet routinely with RN Supervisor to review client cases
  • Document all encounters
  • Participate in On-Call rotation

Required SkillsThe Ideal Candidate for the Client Care Coordinator:
  • Has excellent observation, oral, and written communication skills.
  • Is self-directed with the ability to work with remote supervision
  • Is dependable, organized, flexible and cooperative in fulfilling role obligation.
  • Has integrity and trustworthy
  • Punctual
  • Willing to travel
  • Computer Knowledge

Required Qualifications
  • Is a graduate of an accredited school of professional nursing.
  • Is currently a Licensed Practical Nurse through the North Carolina State Board of Nursing or compact state.
  • Possesses or willing to obtain health clearance including:
    • a negative TB skin test and/or CXR
    • Annual Influenza Vaccine
    • Fully Vaccinated against COVID-19
    • Drug Testing
    • and other tests as required by the organization's policy.
  • Must Possess:
    • Valid Drivers license
    • Access to reliable automobile
    • Car insurance in accordance with state and/or organization requirements
    • CPR certification or willingness to obtain.
  • Must have one or more of the following:
    • Worked at least 1 year as a Certified Nursing Assistant in LTC, Home Care, Home Health
    • Worked at least 1 year as a Licensed Practical Nurse in LTC, Home Care, Home Health, Care Coordinating Capacity
    • Has team lead experience in a health care setting.
