 Job Description: Licenced Rehabilitation Specialist

Reports to  Program Director

Department  Psychiatric Rehabilitation Program

 

 

Summary/objective

The primary responsibility of the mental health Rehabilitation Specialist position under general direction evaluates the social adjustment or vocational needs of persons with mental or emotional disturbances; develop, implement, and monitor rehabilitation plans; act as a consultant for other mental health professionals and agencies; and performs related duties as required.

  • Ensure the delivery of Consumer centered services to all Consumers.
  • Deliver excellent Consumer service toVital Mind  stakeholders.
  • Ensure continuous improvement of theVital Mind ’ behavioral health treatment processes and operations.

 

Job Duties & Responsibilities – Licenced Rehabilitation Specialist

  •  
    • Providing Technical oversight to the Rehabilitation Program to ensure compliance with all COMAR & CARF documentation requirements.
    • Develop diagnostic procedures to determine clients' needs.
    • Prepare and maintain records and case files, including documentation such as clients' personal and eligibility information, services provided, narratives of client contacts, and relevant correspondence.
    • Develop rehabilitation plans that fit clients' aptitudes, education levels, physical abilities, and career goals.
    • Confer with clients to discuss their options and goals so that rehabilitation programs and plans for accessing needed services can be developed.
    • Monitor and record clients' progress to ensure that goals and objectives are met.
    • Arrange for physical, mental, academic, vocational, and other evaluations to obtain information for assessing clients' needs and developing rehabilitation plans.
    • Confer with physicians, psychologists, occupational therapists, and other professionals to develop and implement client rehabilitation programs.
    • Maintain periodic updates of all client plans and assessments as mandated by COMAR.
    • Reviewing and signing off on documentation submitted by Family Service Coordinators for completeness and coaching FSC on documentation improvement when necessary. 
    • Providing technical oversight to group plans and activities to ensure they meet COMAR & CARF standards.
    • Collaborate with clients' families to implement rehabilitation plans such as behavioral, residential, social, and employment goals.
    • Develop and maintain relationships with community referral sources, such as schools and community groups.
    • Foster a staff culture that supports the Vital Mind’s values and codes of conduct.
    • Provide assistance and information to Vital Mind’s management as requested. 
    • Compliance and attendance of scheduled clinical supervision meetings.
    • Compliance with all administrative and clinical policies and procedures including HIPPA.
    • Participation in all scheduled staff meetings and quarterly briefs with the President
    • Other duties as assigned.

Performance Metrics for the Role - Successful completion of probation and ongoing satisfactory performance in the role include the following:

 

  • Ensuring 100% compliance with all COMAR-mandated deadlines on program documentation i.e RRP and all assessments.
  • Ensuring accuracy & completion of all client charts to meet mandated COMAR guidelines.
  • Attending all scheduled Administrative and Supervisory meetings.
  • Attending all scheduled quarterly meetings with the President.
  • Participating and completing all allocated training.
  • To pass probation, all of the following conditions must be met.
  • If all or some of these conditions are unmet, there may be an extension of the probationary period.

 

Supervisory Responsibility

Supervisory Oversight for all Family Service Coordinators

 

Work Environment

This job operates in office and community settings including clients' homes. This role also requires the routine use of standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines. This role is located at theVital Mind  corporate office in Baltimore and also in community settings.

 

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

 

Position Type/Expected Hours of Work

This is a full-time position. This position regularly requires long hours and some weekend work as necessary from time to time.

 

Travel

Travel is primarily local during the business day and this role may require transporting clients in their own transportation.

 

Required Education and Experience 

  

Education: Minimum of a master’s degree in social work, psychology, and mental health/rehabilitation counseling or a related field from an accredited college or university.

 

Special Requirement: In addition to a master’s degree, candidates must have a valid license as a Licensed Graduate Social Worker or Professional Counselor [LGPC or LGSW] or a Licensed Clinical Social Worker or Professional Counselor [LCSW-C or LCPC]. 

 

Experience:  At least one year of professional experience working in mental health rehabilitation counseling and related social services in a public or certified private welfare or health agency.
