How you will make a difference
- Provide assistance in the home with Activities of Daily Living,assistance with household cleaning and the maintenance of a safe home. Provide assistancein the home with routine household chores. Provide a nutritious and well-balancedmeal prepared at or delivered to the home.
- Meetphysical and sensory requirements stated below and be able to work in thedescribed environment.
- Identifyand participate in process improvement initiatives that improve the customerexperience, enhance workflow, and/or improve the work environment.
- Providepersonal care and assistance with Activities of Daily Living (ADL) (i.e.,grooming, bathing, feeding, toileting, oral care, nail care, dressing, etc.),as indicated by the treatment plan.
- Assistwith homemaker services/routine household chores (i.e., dusting,sweeping/mopping floors, washing dishes, ironing, cleaning kitchen andbathrooms, providing laundry service, changing bed linen, etc.), as indicated.
- Plan,prepare, deliver meals, grocery shopping.
- Visit clients in theirhomes to review client satisfaction and obtain meal orders.
- Package meals,transports meals to clients via personal or company vehicle and delivers mealsto client homes.
- Record and track deliverieson Meal Delivery Sheet.
- Utilize proper bodymechanics when lifting or transporting meals.
- Take and record foodtemperatures as required per local and state guidelines.
- Design meal deliveryroutes to ensure efficiency.
- Place bulk meal ordersvia DSSI and maintain meal inventory.
- Completeerrands (i.e., getting a prescription filled, etc.).
- Providecompanionship to clients.
- Followassignment sheet for tasks.
- Ambulateand transfer clients, utilizing appropriate assistive devices and body mechanics.
- Providereminders to take medication and loosen the cap (do not give medications).
- Completeproper documentation (date and place service provided, times service provided,actual service provided, etc.).
- Observeclients carefully and report changes in condition.
- Recognizeclient's ability to make choices and support these within the constraints ofcare giving environment.
- Assistwith the maintenance of a safe environment.
- Providerespite care, on a short-term basis, when there is an absence of, or need for,relief for the caregiver.
- Assistwith obtaining referrals.
- InstallPERS as backup.
- CompleteAide Visits and Checklists with-in 30 days of hire and annual.
- Assistwith development of Care Plans.
- Attendand participate in Safety Committee Meetings as needed.
- Mentorand provide training to other Home Care Aides.
- Provideinput into performance evaluations for other Home Care Aides.
- Attendtrainings/in-services as required.
- Participatein the Quality Assurance and Quality Improvement plan processes.
- Other special projects and duties, asassigned. Such as assist with admissions, new staff introductions to clients,collect completed SDR's, mileage and travel forms for delivery to office.
- Maintaintwelve (12) hours of in-service training each year spread across four (4)quarters in addition to normal supervisory guidance and training, AND
- NoCoaching & Counseling's for attendance, AND
- Atleast 90 days of employment with the company with an excellent evaluation(s).
- If operatinga personally owned vehicle to go from one client to another, you must have avalid State Driver's License, a good driving record and current auto insurancethat meets the state minimum.
- Highschool diploma or GED, preferred.
- Effective verbal and written Englishcommunication skills.
