Lead Gender Health Clinician

The Lead Gender Health Clinician (GHC) serves as a leader for One Community Health's Gender Health (GH) program, working in conjunction with the HIV/GH Division Lead and HIV Program Director. The Lead GHC will serve as subject matter expert for GH for OCH providers and staff. The Lead GHC will provide direct care to adult and pediatric transgender and gender-nonbinary (TGNB) patients, serving as their primary care provider. In some circumstances, the Lead GHC may serve in a consultation role while the patient continues with their internal or external PCP. In addition to primary and gender affirming care (GAC), the Lead GHC will provide HIV treatment and prevention services.


Essential Functions

Administrative

  • Serve as subject matter expert for ongoing development and implementation of the GH Program's protocols, policies, and best practices, ensuring the delivery of cutting-edge, compassionate care.
  • Design and facilitate semi-annual trainings for clinicians, while sharing valuable clinical insights at monthly meetings, fostering a culture of continuous learning and professional growth.
  • Utilize your deep understanding of healthcare outcomes and quality measures to monitor and optimize the well-being of our TGNB patients, championing ongoing quality improvement initiatives.
  • Drive the ongoing development and expansion of our gender-affirming services, ensuring that we continue to meet the evolving needs of our diverse patient population.
  • Forge strong partnerships across departments, including Clinical Pharmacy, to work together to refine and enhance our GH Clinical Practice Guidelines and ensure the best possible care for our patients.
  • Maintain a steadfast commitment to excellence in care, conducting periodic chart reviews, case conferences and peer reviews to assess the quality of treatment provided to our GH program patients and identifying opportunities for improvement.
  • Provide personalized guidance and mentorship to residents and staff, equipping them with the skills, knowledge, and confidence to excel in their roles and contribute meaningfully to the care of TGNB patients.

Clinical

  • Provide comprehensive primary and consultative care to adult and pediatric TGNB patients, including initiation and management of gender-affirming hormone therapy and puberty blocking medications, referral to gender-affirming surgeries, HIV treatment, and HIV prevention.
  • Skillfully diagnose and treat acute illnesses, as well as chronic diseases and conditions such as hypertension, heart disease, and diabetes, employing your expertise to optimize patient outcomes.
  • Serve as subject matter expert for GAC for OCH providers and staff being available for formal and informal case consultations as needed.
  • Clearly communicate with patients about their treatment plans, thoughtfully explaining procedures, test results, and prescribed therapies, fostering understanding and trust.
  • Attentively track patients' conditions and progress, adapting treatments as necessary and ensuring optimal care at every stage of their journey.
  • Exercise sound judgment in prioritizing medical care, collaborating with specialists as needed to address high-risk or complex cases, ensuring comprehensive support for each patient.
  • Uphold a steadfast commitment to excellence, ensuring top-quality medical care is delivered to all patients and fostering a culture of continuous improvement.
  • Demonstrate a thorough understanding of medical clinic policies and relevant state and local regulations, ensuring compliance, appropriate patient follow-ups, and specialty referrals.
  • Understands and monitors systems for compliance as necessary. Provides necessary and timely communication to Medical Leadership when issues of compliance arise and complies with a corrective strategy.

Additional Duties

  • Participate in meetings, work groups, case conferences, task forces and committees to support ongoing clinic functions.
  • Foster communication and coordination within the clinic and outside agencies by establishing professional working relationships with others to facilitate the delivery of services to the patient.
  • Assist with training, orientation and clinical support for Fellows, new providers, advance practice providers, and clinical support staff.
  • This position is responsible for reporting suspicion of child, adult (including spouses), elder, and dependent adult abuse as outlined in Penal Code, Section 11166, Section 15630, and Section 1160
  • Other duties as assigned.

Minimum Requirements

  • Must be fully licensed to practice in the state of California, who is board certified
  • Board Certified in Family Medicine or Internal Medicine with experience in gender affirming care (MD, NP, PA, or DO)
  • Possess a DEA certificate
  • Active CPR, BLS Certification.
  • Licenses required for this position must be kept current, valid, and in good standing with the presiding entity.
  • Driver's License- possible travel between locations
  • Expertise in the following areas, typically gained from 3+ years:
  • Providing care for transgender and gender nonbinary patients;
  • Experience in a clinical leadership role;
  • Providing HIV prevention (PrEP) and care for patients living with HIV;
  • Providing care for people with varying disabilities, racial, ethnic and cultural backgrounds and economic statuses; and
  • Experience working with people who do sex work, experiencing homelessness, challenges with mental illness and/or are using substances, those of with a history of incarceration, and people of varying ages.

Preferred

  • American Academy of HIV Specialist Certification
  • Experience with EPIC electronic medical records
  • Bilingual in Spanish
