Job Overview:
Home Matters Caregiving is seeking a compassionate and dedicated individual to join our growing team. This position supports our dedication to caregivers and our caregiver first philosophy by providing caregivers with a mentor, coach and leader. The Lead Care Manager will guide their team from orientation and training through to the first shift introduction and client transitions. They also serve as a liaison between caregiver and client by conducting in-home assessments, developing care plans and helping caregivers provide world class service.
This position bridges the gap between caregiver and client in this dynamic home care environment. Not only will the Lead Care Manager provide leadership and support for caregivers but they will also cover shifts for call outs and other availability issues when needed.
This is a dynamic and rewarding position focused on the most important part of the home care business - The Caregiver.
Key Responsibilities:
- Conduct initial assessments of clients referred for non-medical in-home care services.
- Develop comprehensive care plans tailored to each client's specific needs and preferences.
- Performs in-person client reassessments as scheduled and notifies the main office of any changes in the client's condition that may necessitate changes to the established care plan.
- Regularly reviews and updates care plans as needed to ensure the highest quality of care.
- Leads a team of caregivers to provide guidance and support.
- Maintain accurate client records and documentation.
- Collaborate with the administrative team to optimize staffing efficiency.
- Conducts orientation, onboarding and training new caregivers as needed.
- Participates in daily staff and team meetings.
- Covers open caregiving shifts in the event of call outs or availability.

- CNA or LPN helpful but not required.
- At least (3-5) years of previous experience with in-home health care is helpful.
- Proficient in Microsoft office.
- Experience using Wellsky a plus.
- Must have the use of an automobile in good working condition, a valid driver's license, and automobile insurance compliant with company policies.
Job Type: Full-time
Pay: $52,000.00 - $63,000.00 per year
