Description
We are looking for a Lead Care Manager to join our team in the ECM Program. As the Lead Care Manager, you will be responsible for overseeing the care management team, ensuring that high-quality care is provided to our clients.
Responsibilities:
- Oversee the care management team and ensure that all team members are properly trained and equipped to provide high-quality care to our clients
- Develop care plans for clients and ensure that they are followed by the care management team
- Monitor the progress of clients and adjust care plans as needed
- Coordinate with other healthcare professionals to ensure that clients receive comprehensive care
- Act as a liaison between clients and their families, providing support and guidance as needed
- Conduct regular assessments of clients' needs and make recommendations for changes in care plans
- Maintain accurate and up-to-date records of client care and progress
Performs appropriate documentation to maintain the standards set by VCH, with special attention to the ECM assessment and care plan.
Collects and prepares reports and documentation as needed for planning and quality assurance.
Participates in health center and department quality improvement initiatives.
Maintains confidentially and complies with HIPAA and compliance mandates at all times.
Requirements
Requirements:
- Bachelor's degree in nursing, social work, or a related field
- Minimum of 5 years of experience in care management
- Strong leadership skills and the ability to motivate and inspire a team
- Excellent communication and interpersonal skills
- Knowledge of healthcare regulations and best practices
- Ability to work well under pressure and in a fast-paced environment
- Strong problem-solving and critical-thinking skills
