Job description

Department:  COMCARE

Pay:  $25.36 per hour. | This position may also be eligible for a sign on bonus of $2,500 after 60 days of employment if selected applicant meets all policy conditions.  

Work Schedule: Monday - Thursday, 7:00am - 6:00pm with varied hours

Process referrals, conduct pre-release assessments, and link clients with mental health and substance use services during the re-entry process.  Collaborate with Kansas Department of Corrections staff to coordinate services prior to re-entry. Provide intensive case management to reduce revocations in close coordination with Kansas State Parole Office. Assist consumers in accessing community resources to address basic needs. 

 

Direct Service Provision

  • Coordinate and engage in pre-release contact with all individuals referred from the following Correctional Facilities: El Dorado, Hutchinson, Winfield, Larned, Topeka, Norton, and other facilities as requested in order to connect clients to appropriate mental health services
  • Provide assertive outreach and case coordination for assigned clients to aid client in achieving goals identified in the individualized treatment plan; outreach interventions include: treatment planning, collateral contacts, phone support, access to supports and service coordination
  • Provide medically necessary interventions to clients
  • Provides transportation to support client attendance in medical appointments and other therapeutic goal related activities to include development of community resources, employment or education access, social or consumer-run activities and related supports
  • Conduct risk assessments as needed to address safety of clients
  • Coordinate with Parole Office staff to achieve program outcomes

 

Treatment Planning

  • Review intake assessment completed by QMHP for all assigned clients
  • Complete initial treatment plan and crisis follow-up plan with client and treatment team members no later than 30 days from the date of the intake
  • Conduct treatment plan review and update treatment plan documenting progress toward established treatment goals every 90 days following the date of intake or as needed to reflect client’s progress and treatment needs
  • Coordinate with other service providers to review client’s progress and update treatment plan goals, objectives and interventions

 

Documentation

  • Complete progress notes in accordance with COMCARE and CCBHC guidelines which meet requirements for medical necessity, goal-directed treatment and client response to treatment within set timelines
  • Complete progress notes within 48 hours of service delivery with/or on behalf of the client
  • Prepare court reports as needed and feedback for Mental Health Court Proceedings
  • Complete Notes for Record, releases of information, safety documents and other documentation as required and/or recommended within established deadlines
  • Track and complete monthly outcome data
  • Respond to Utilization Review staff feedback forms and make corrections as needed
  • Enter AIMS data monthly
  • Completes discharge summary when closing to case management services

 

Minimum Qualifications: Bachelor’s degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education. If no degree is present applicant may substitute each one year of experience for one year of education. Must have a valid Kansas Driver’s license and current proof of automobile insurance. Must be able to safely operate a motor vehicle and meet driver qualification standards as set forth in Sedgwick County’s Fleet Vehicle Operation and Usage policy.  Meet the specifications as outlined in the CMHC/CCBHC licensing standards and pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.  Must complete orientations provided by Sedgwick County and COMCARE.

 

Preferred Qualifications: Bachelor’s Degree.
