Requirements: Up-to-date Coaching Permit, CPR, First Aid and proof of successful concussion training. All coaches must take a 5 year refresher on concussions. All coaches must annually review the state awareness plan on sudden cardiac arrest.
Inquiries regarding obtaining a Temporary Coaching Permit may be obtained by calling or emailing the Athletic Director, Michelle Pac.
Please indicate whether your Coaching Permit, CPR, and First Aid are current, if so, what are the expiration dates.
Application Procedure: Apply Online - Employees of Windsor Locks, use internal application only.
