Pacific Clinics serves children, transitional age youth, families, adults, and older adults. POSITION SUMMARY:
Responsible for developing job leads and placing adults with mental illness in competitive employment positions in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Meets with employers to develop jobs for individual placements of consumer workers.
  • Assesses vocational and related needs of consumers.
  • Develops and implements an individual service plan with consumer and Department of Rehabilitation Counselor.
  • Provides consumers with assistance in developing the following skills:

Interview Techniques
Individualized, Job Specific Resume Development
Master and Job Specific Application Preparation
Appropriate Work Behaviors/Etiquette
Grooming and Hygiene
Exploration of Job Interests
  • Conducts weekly Job Club meetings with consumers ready for work.
  • Accompanies consumers to placement interviews and work sites as needed.
  • Provides on and off-site job coaching as needed to support consumers.
  • Coordinates services with other Employment Services Team Members, Rehabilitation Counselor and Clinical Case Manager to ensure the consumer has a comprehensive support network.
  • Acts as primary contact/liaison with employers and provide mediation when necessary.
  • Monitors consumer progress and complete monthly evaluations.
  • Maintains program and consumer records and files, provides reports and other documentation as necessary to fulfill position and accreditation requirements including, but not limited to consumer database, log of employer contacts, and record of placements.
  • Attends all required meetings and in-service training.
  • Respects confidentiality and all other rights of consumers.
  • Maintains Red Cross Certification in First Aid and CPR.
  • Interacts with other internal programs, clinical/support staff and external sources such as school or community personnel as applicable.
  • Reports to work on time and maintains reliable and regular attendance.
  • Models Pacific Clinics' approach, mission and core values in all communication and correspondence.
  • Communicates effectively in a culturally competent and diverse consumer population and promotes favorable interaction with managers, co-workers and others.
  • Performs other duties as assigned.

QUALIFICATIONS:

ORGANIZATIONAL RELATIONSHIPS/INTERACTION:
Initiate and maintain professional interactions and communication with Clinic's employees and/or others.
EDUCATION and/or EXPERIENCE/POSITION REQUIREMENTS:
  • Bachelor's Degree in Rehabilitation Counseling or other related field preferred.
  • Minimum of three (3) years of experience providing employment rehabilitation services for the mentally ill or a demonstrated ability.
  • A working knowledge of the community job market,
  • Demonstrated skills working effectively with the homeless and dually diagnosed population.
  • General computer skills including Microsoft Office software.
  • Effective organization skills and ability to work with minimal supervision.
  • Ability to work effectively as a team player.
  • A working knowledge of the Psychosocial Rehabilitation model, preferred.
  • Basic knowledge of public policies relative to member population and associated programs, preferred.

  • Effective verbal and written communication skills, bi-lingual skills in Spanish/English preferred.
  • Must possess a valid California driver's license and maintain an insurable driving record under the Clinics' liability policy.
  • Required to obtain First Aid and CPR certification within first six months of employment and Non-Violence Intervention certification within first year of employment with the Clinics. First Aid and CPR certification must be maintained throughout employment with the Clinics.
  • Ability to work effectively with diverse member population and amongst a diverse staff.
  • Ability to work a flexible schedule as needed to fulfill program needs, which may include weekends, evening, and holidays.

PHYSICAL DEMANDS:
While performing the duties of this job the employee is frequently required to stand or sit. The employee is required to use hands to produce records and/or documentation in manual or electronic format. The employee must regularly lift and/or move up to 5 pounds and occasionally move or lift up to 10 pounds.

While performing the duties of this job, the employee is occasionally exposed to moving mechanical parts and outside weather conditions. The noise level in the work environment is usually moderate. Community outreach and local business travel is required.
