GENERAL SUMMARY:

Under general supervision of the Program Manager, Housing Services and according to The Health Trust’s (THT) general policies and procedures, the Intake Support Specialist is responsible for supporting clients in the process to obtain and ultimately retain housing and facilitate access to HIV case management supportive services. The position performs confidential, precision-based administrative tasks; assists clients in the initial rental subsidy application process and initial HIV case management intake and enrollment process; conducts detailed client interviews to obtain complete and accurate information; and maintains available community resources to provide to clients as appropriate. This position works collaboratively with benefits counselors, case managers, social workers, nurses, and others in the community. This is an office position and remote work is not available.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Coordinates and maintains an up-to-date interest list and referral log.

  • Requests and collects all the necessary documents to determine eligibility for internal programs.

  • Interviews applicants, elicits personal and financial information and provides assistance in matters such as completing forms and obtaining required verifications to determine eligibility and benefit level for program assistance initially.

  • Completes and reviews paperwork requiring a high degree of attention to detail and accuracy.

  • Documents information and any requests received from clients, landlords, clinics, and other community stakeholders, such as client referrals for the housing program interest list and HIV Services Program.

  • Maintains adequate records and other documentation necessary for the collection of data and statistics pertaining to program outcomes, demographics, and information as required by funders.

  • Completes and processes background checks for interest list applicants.

  • Contacts individuals, public and private agencies, businesses, and other sources to verify eligibility data, to clarify documentation discrepancies, and/or gaps in information.

  • Communicates effectively with a wide variety of persons including difficult individuals, clients/families, team members and others as appropriate as well as consults with other staff regarding client referrals to appropriate resources.

  • Provides screening, information and referrals, intake and assessment, crisis management to potential clients seeking services or information regarding the program services.

  • Accurately completes, tracks and distributes intakes/referrals for applicable program(s) in a timely manner.

  • Maintains accurate knowledge about the agency, its programs and services.

  • Accurately represents the agency and maintains positive working relationships with the general public, clients, funding sources, referral sources, etc.

  • Attends and participates in client care meetings, HIV Services and Housing team meetings and provides updates as needed on clients’ progress in the housing and HIV services intake process.

  • Performs administrative tasks required for the job including data entry, word processing, typing, filing, and answering phones.

  • Maintains all HIPAA standards of privacy and confidentiality.

  • Other duties as assigned.

EXPERIENCE AND TRAINING

  • Bachelor’s degree from an accredited school or equivalent combination of education, training, and experience.

  • Experience and knowledge of subsidized Housing programs and the local housing market.


PREFERRED EXPERIENCE & TRAINING

  • Bilingual preferred.

MINIMUM KNOWLEDGE AND ABILITY

  • Ability to understand and adhere to the rules and regulations governing HUD-funded housing programs and Ryan White benefits.

  • Ability to understand and adhere to the pertinent Federal, State, and local housing laws, codes, and regulations.

  • Ability to conduct effective interviews at times under difficult or stressful circumstances.

  • Knowledge of business correspondence and ability to write clearly and effectively and to proofread for grammar, spelling, and punctuation with a high degree of accuracy.

  • Ability to understand, learn and disseminate knowledge of community resources in Santa Clara County.

  • Ability to perform basic math such as addition, subtraction, multiplication, division, and calculating decimals, percentages, and fractions.

  • Ability to work independently and/or with general supervision.

  • Ability to prioritize, problem solve, and utilize critical thinking and good judgment skills.

  • Knowledge of principles of research and data collection.

  • Excellent written and oral communication skills.

  • Ability to work effectively with people of diverse cultures, ages and economic backgrounds in a culturally competent and sensitive manner.

  • Demonstrated proficiency in Microsoft Suite and Google products

  • Self-motivated and demonstrated critical thinking skill

  • Demonstrated strong teamwork and people skills, with ability to develop productive relationships

  • Ability to model and communicate the mission of The Health Trust and support a culture of empowerment and team support.

WORKING CONDITIONS:


  • Normal office environment with little exposure to excessive noise, dust, and temperature.

  • Car available for daily use with valid driver’s license and automobile insurance as required by the State of California.

  • Employment background screening required.

  • Annual Tuberculosis testing.


REPORTING RELATIONSHIPS: Program Manager, Housing for Health & Housing

Pay Rate: $28.29/HR - $31.25/HR
