Position Overview:

We are currently seeking a dynamic and experienced Intake Supervisor to lead our intake department. The Intake Supervisor will play a crucial role in overseeing the intake process, coordinating referrals, and ensuring a seamless transition for clients into our home health services. This position requires a detail-oriented individual with excellent communication skills, a strong understanding of home health regulations, and the ability to collaborate effectively with internal teams and external stakeholders.

Responsibilities:

Intake Coordination: Manage the intake process, including receiving and reviewing referrals, verifying eligibility, and coordinating assessments with clinical staff.

Supervision of Intake Specialists: Provide leadership and guidance to the intake specialists, ensuring their adherence to protocols, timely response to referrals, and accurate documentation.

Documentation and Compliance: Ensure all intake documentation is complete, accurate, and compliant with regulatory requirements. Stay current on industry regulations and updates.

Communication: Collaborate with physicians, case managers, and other healthcare professionals to facilitate the intake process. Maintain clear communication channels with internal departments.

Team Collaboration: Work closely with clinical and administrative teams to facilitate a smooth transition for clients from intake to the initiation of home health services.

Quality Assurance: Implement quality assurance measures to monitor and enhance the efficiency of the intake process. Identify areas for improvement and implement solutions.

Training and Development: Provide training to staff involved in the intake process, ensuring they are well-versed in policies, procedures, and compliance standards.

Qualifications:

Active LPN or RN license.

Minimum of 5 years of experience in home health intake coordination or a related role.

Operations management experience.

Strong knowledge of home health regulations and compliance standards.

Strong work ethic and excellent communication and interpersonal skills.

Proficient in relevant software applications such as Home Care Home Base and electronic medical records (EMR).

Detail-oriented with strong organizational and problem-solving abilities.

Leadership skills and the ability to work collaboratively in a team-oriented environment.
