Position Summary
The Intake and Eligibility Coordinator will be responsible for welcoming new potential Care Coordination Organization (CCO/HH) Enrollees and assisting them through the OPWDD Eligibility Process so they can ultimately receive Care Management services. Specifically, you will assist any interested and/or referred individuals through the Office of People with Developmental Disabilities (OPWDD) eligibility process as well as ensure appropriate benefits coverage (Waiver Medicaid) are in place for enrollment. This position will enable individuals to potentially receive lifelong benefits associated with needed Medicaid Services as well as behavioral, medical, and social Programs to increase and maintain the quality of an individual's life.

Primary Duties and Responsibilities:
  • Interpret and clarify state policy (OPWDD/ DOH) to assure accurate and timely processing of required eligibility documentation;
  • Collect and analyze intake data to evaluate the effectiveness and efficiency gaps in the eligibility process;
  • Participate in local policy development and recommend state policy changes;
  • Responsible for coordination of initial intakes with the Administrative Assistant to ensure timely response to families and individuals;
  • Analyze problem situations and adopt an effective course of action;
  • Prepare and present clear and concise oral and written reports;
  • Provide presentations to outside agencies on Southern Tier Connect, the intake process and available OPWDD services
  • Establish and maintain effective working relationships with potential members, colleagues, public partners and those contacted in the course of work;
  • Perform other job duties requiring skills, knowledge and physical requirements as demanded by those duties described or less. Individual assignments will be determined by the supervisor based on then current workloads and department needs;
  • This position will handle all intake processes, after initial contact, through Eligibility and Medicaid determination, up to and including enrollment into this CCO/HH. The responsibilities will include conducting the intake, collecting any appropriate documentation to facilitate the intake process, providing information related to STC and State processes, completing paperwork, attending planning meetings as appropriate, and making home visits to complete intake packet or collect any necessary documents to aid in the expeditious processing of the eligibility packets;
  • Facilitate Assessments for expedited enrollment in HCBS Waiver when applicable
  • Responsible for checking with local DSS and/or regional Medicaid Offices for Medicaid eligibility for the individuals who are in process for OPWDD eligibility;
  • Understand benefit and entitlement programs. Being able to understand and work within Southern Tier Connect's chosen EHR system,
  • Work closely with STC Supervisors and Care Managers to ensure families and individuals are assigned to Care Managers prior to CCO enrollment;
  • Understand company caseload structures, transfer protocol between CCO's and importance of customer service and continuity of services;
  • All other duties as assigned.

Qualifications, Skills and Knowledge Requirements:
  • Bachelor's degree with two years of relevant experience, (required) or
  • A license as a Registered Nurse with two years of relevant experience, which can include any
  • employment experience and is not limited to case management/service coordination duties, or
  • A Master's degree with one year of relevant experience.
  • Must be able to work a flexible schedule.
  • Driver's License
  • Duties require professional verbal and written communication skills.
  • Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
