  • JOB SUMMARY AND SPECIFICATIONS
    • JOB SUMMARY
      • The FIM Transitional Care Clinical Pharmacist will provide comprehensive, value-based, medication-related care for a wide variety of patients with chronic health conditions before their discharge from the inpatient setting.  
    • MINIMUM QUALIFICATIONS
      • Professional & Technical Skills
        • Good written and oral communication skills and math ability (including ability to work with mathematical concepts such as probability and statistical inference as well as the ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations). Ability to operate various types of office equipment, including fax machine, computer, and calculator.
      • Education
        • PharmD or Bachelor's in Pharmacy Degree from an ACPE-accredited school of Pharmacy.
      • License(s) or Certification(s)
        • State licensure as a Pharmacist through the State of Indiana. BCACP, BCPS, CDCES, or BC-ADM preferred.
      • Other Skills or Requirements
        • Must be knowledgeable about optimal medication use in the following age-related patient groups: Neonatal, Pediatric, Adult, Geriatric.
      • Skilled to Care for Certain Age-Related Patient Groups (incumbents will be skilled in the care of the following patient groups)
        • All age groups
