Responsibilities include: Providing guidance and assistance for the Independent Living (“IL”) residents, their families, resident representative (RP) or Power of Attorney (POA) and acts as a liaison to IL residents (etc.) with the facility policies and procedures. Refers IL residents to social, health and community agencies as needed to solve financial needs and promote emotional security. Documents appropriately in the individual resident record his/her interactions with residents and with family members. Collaborates with Resident, Resident Family or RP, IL Resident Health Care Coordinator, Spiritual Wellness Coordinator, Director of Nursing, Household Coordinator and Health Care Social Worker to see that resident is placed in the appropriate level of care for his/her needs according to facility policies when a change in resident’s condition is identified. Supports residents in need of emotional, financial, physical, spiritual, social service programming.
Full-Time Monday-Friday 1shift with flexibility required

Required:
- Degree in Social Work or related field required
- Experience working with elderly, Long Term Care experience desired
- Knowledge/familiarity with insurance basics
- Must have excellent communication skills
- Has ability to work with computers and other electronic equipment
- Training and experience in counseling skills, preferred
- Teaching and/or public speaking experience, preferred
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity
