The Immunization Coordinator promotes a strong belief in Adapt Integrated Health Care's philosophy, purpose, and ideals. A health care professional who is responsible for overseeing current clinic vaccine programs in accordance with clinic policy by coordinating and administering childhood and adult vaccines as indicated and documenting/reporting/tracking administered vaccines.
Primary Responsibilities:
- Coordinating the scheduling pediatric patients for provider appointments in conjunction with Immunization schedule with the teams at all locations.
- Take pediatric/triage calls when needed.
- Coordinate/administer majority of childhood and adult immunizations when able.
- Track/review/send reminders out to all pediatric patients when they are due for immunizations
- Order and track all administered immunizations.
- Complete vaccine inventories for both VFC and private stock monthly.
- Ensure stock injection supplies are always adequate.
- Participate and manage any potential research/community programs for immunizations as indicated.
- Participate in community vaccine programs.
- Attend any continuing education or update meetings/webinars pertinent to update vaccine knowledge.
- Update P&P and VIS reports as indicated.
- Maintain yearly/quarterly vaccine reports.
- Fill in for MA's/PCC's as needed.
- Work in conjunction with QI Manager doing chart review on pediatric patients to ensure health maintenance is up to date.
- Data load historical vaccine history as needed.
- Maintain pediatric UDS/CCO measures and report.
Required Qualifications:
- Current CPR certification
- Current Oregon Driver's License.
