From homelessness to supportive housing, Lakeshore CAP provides housing programs, support, and solutions to address the housing needs in our community. The Supportive Housing Case Manager works with those that are found to be literally homeless or at imminent risk of homelessness and provides referrals and supportive services.
Job Outline:
· Performs intake, confirms eligibility and provides case management services using a trauma informed approach. Assists households in developing personalized goal plans, monitors and supports progress of participants while providing individual case management. Refers participants to relevant programs through LCAP and/or partners.
· Enters data in agency tracking system. Runs necessary and applicable reports within data systems.
· Participates in an interdisciplinary approach to client situations seeking out practical and innovative solutions to problems presented by working with other agency partners to address issues found in common among clients and programming.
· Represents the program in various professional groups and potential collaborations.
· Participates in the semi-annual Point in Time Count to conduct outreach to those without shelter.
· Complies with all confidentiality requirements; regular supervision, agency staff meetings, training sessions, networking meetings and other events as directed.
· Maintains positive relationships with program recipients, service providers, volunteers, staff and community members.
Job Requirements
- Must be passionate about Lakeshore CAP’s mission.
- BA/BS in a social work related field or equivalent work experience preferred.
- Excellent computer skills in a Microsoft Windows environment
- Ability to maintain a high level of confidentiality, a professional demeanor to represent the organization
- Must be able to pass a criminal background check
- Must have a reliable vehicle, valid Wisconsin driver’s license and current personal auto insurance as required by law.
- Understands and implements or is willing to learn a Trauma Informed Care approach.
- Must possess strong oral and written communication skills, be dependable and adaptable.'
