Position:  Housing Stability Outreach Coach

Location: York and Lancaster County

Essential Functions for the Housing Stability Outreach Coach

  • To provide street-based outreach in Lancaster to youth who live or spend a significant amount of time on the street or in other areas of exposure which place them at risk of abuse, trafficking and exploitation. Successfully engage youth/client in developing and implementing their individual goal plan using positive youth development, trauma-informed and culturally appropriate frameworks in the York Rapid Rehousing program.
  • Ensure the timely & accurate completion of all required paperwork per the LSI Paperwork Guidelines document.
  • Provide a minimum of 17 hours a week of 1-on-1 sessions/credible contacts with clients identifying barriers and connecting to resources to alleviate those barriers. A focus on harm-reduction in the street outreach efforts is imperative.
  • Be the primary liaison between the Coalition of Homelessness teams and Valley Youth House.

Minimum Qualifications for the Housing Stability Outreach Coach

  • An associate's degree in Social Work other related field. If the degree is in an unrelated field, then the candidate must have at least 1 year of experience in the social services field.
  • One or more years' experience working with youth and young adults or in case management or street outreach.
  • A valid driver's license, a motor vehicle report that meets agency requirements and reliable transportation that can safely transport clients if needed.
  • Ability to work a hybrid schedule that includes evenings.
  • Ability to complete the pre-employment physical and drug test requirements.

Preferred Qualifications

  • One to two years of experience doing street outreach or case management with individuals experiencing homelessness.
  • Knowledge of the services available in both York and Lancaster Counties.
  • An understanding of the barriers and challenges that individuals experiencing homelessness face, especially those unique to the 18-24-year-old population.

Physical Requirements:

  • Sedentary work that primarily involves sitting/standing. X Occasionally
  • Light work that includes moving objects up to 20 pounds. X Occasionally
  • Intense work that requires walking and maneuvering around obstacles X Occasionally
