Position: Housing Stability Outreach Coach

Essential Functions for the Housing Stability Outreach Coach
- To provide street-based outreach in Lancaster to youth who live or spend a significant amount of time on the street or in other areas of exposure which place them at risk of abuse, trafficking and exploitation. Successfully engage youth/client in developing and implementing their individual goal plan using positive youth development, trauma-informed and culturally appropriate frameworks in the York Rapid Rehousing program.
- Ensure the timely & accurate completion of all required paperwork per the LSI Paperwork Guidelines document.
- Provide a minimum of 17 hours a week of 1-on-1 sessions/credible contacts with clients identifying barriers and connecting to resources to alleviate those barriers. A focus on harm-reduction in the street outreach efforts is imperative.
- Be the primary liaison between the Coalition of Homelessness teams and Valley Youth House.
Minimum Qualifications for the Housing Stability Outreach Coach
- An associate's degree in Social Work other related field. If the degree is in an unrelated field, then the candidate must have at least 1 year of experience in the social services field.
- One or more years' experience working with youth and young adults or in case management or street outreach.
- A valid driver's license, a motor vehicle report that meets agency requirements and reliable transportation that can safely transport clients if needed.
- Ability to work a hybrid schedule that includes evenings.
- Ability to complete the pre-employment physical and drug test requirements.
Preferred Qualifications
- One to two years of experience doing street outreach or case management with individuals experiencing homelessness.
- Knowledge of the services available in both York and Lancaster Counties.
- An understanding of the barriers and challenges that individuals experiencing homelessness face, especially those unique to the 18-24-year-old population.
Physical Requirements:
- Sedentary work that primarily involves sitting/standing. X Occasionally
