What You'll Do
The Journey Health Housing Navigator will be responsible for connecting eligible members to the Journey Health Housing Program. The Housing Navigator will work with a multidisciplinary team to complete Housing Deposit referrals and facilitate members through the Housing Transition Navigation Services and Tenancy Sustaining Services as outlined by DHCS and CalAim. Journey Health’s Housing Navigator will be integral in providing long-term solutions for improved housing.
Responsibilities
- Communicate effectively and timely with landlords on behalf of the member.
- Maintain multi-stakeholder relationships with various departments (county health, public health, substance use, mental health, and social services departments)
- Complete housing assessments to determine needs and gaps in services.
- Guide members through completion of housing related documents, organize applications, and provide coaching for interviews.
- Create individualized housing support plans.
- Coordinate housing related needs including communication with landlords and various housing departments.
- Identify appropriate housing for various needs and financial circumstances.
- Obtain resources to assist members with utilities.
- Conduct field visits to determine that potential housing environments are safe and available for the member.
- Arrange all logistics and notify members of the moving plan.
- Maintain accurate documentation utilizing required technology as outlined by Journey Health
- Perform other related activities as assigned.
What You Need
Qualifications
- Experience navigating the healthcare system and housing support.
- Demonstrates experience in providing culturally sensitive care and delivering high-quality customer service to diverse communities.
- Communicates clearly and effectively with team members, clients, and stakeholders.
- Work independently, make informed decisions, and manage tasks efficiently in a fast-paced environment.
- Support colleagues, cross-train on diverse job functions, and foster a cohesive, high-performing team.
- Embrace organizational changes in a dynamic environment while remaining adaptable and proactive.
- Proficiency in Microsoft Suite applications (Word, Excel, PowerPoint, Outlook, and Teams)
- Familiarity with motivational interviewing, clinical documentation, Medi-Cal and Medicare
- Knowledge of policies related to the housing crisis.
