General Description

The Housing Liaison provides programmatic support for all Places for People housing subsidy programs. The individual filling this position works collaboratively with PfP staff, the Institute for Community Alliances, Continuum of Care, Behavioral Health Network, as well as landlords in the community to expedite housing placements. Such collaboration will include providing housing navigation assistance to clinical teams for new and existing clients. These activities include but are not limited to assisting with placement on the prioritization list, tracking contacts every 90-days, updating system with re-certification information, and completing VI-SPDAT with the client and entry into Service Point's Homeless Missourians Information System.

Job duties include but are not limited to:

  • Practices Housing First and other low barrier service delivery models
  • Attends local Continuum of Care, Behavioral Health Network, Institute for Community Alliances and other related meetings in the community
  • Receives referrals from Continuum of Care programs to assist individuals with housing stability
  • Respects agency policy, including confidentiality of persons served
  • Empowers others to solve their own problems
  • Recruits landlord and builds rapport by directly contacting potential landlords to provide information and make referrals to their units. Follows up with landlords about referrals and problem solves tenant issues to maintain rapport
  • Collaborates with teams and departments within the agency as it pertains to helping persons served (clients) meet their housing needs
  • Completes assessment (VI-SPDAT) with persons served and enter into HMIS Service Point
  • Educates teams and departments about the Coordinated Entry / Prioritization List process
  • Communicates updates from Continuum of Care, Behavioral Health Network and Department of Mental Health with appropriate PfP staff
  • Maintains accurate documentation and updating in Service Point and the Housing Department's databases
  • Creates and maintains system to assist with tracking person served on Prioritization Housing / Surge list and follows up with teams as needed to ensure person served remain in active status on both lists
  • Coordinates documentation requirements for person served with housing opportunities with staff
  • Maintains tenant files, ensuring information is accurate and up-to-date, and follows documentation requirements for specified housing programs.
  • Conducts housing inspections as needed
  • Answer questions from staff and provide housing resources as needed
  • Completes annual training requirements as outlined by agency requirements and supervisor
  • Performs other related tasks as assigned by the supervisor
