ElizabethCoalition to House the Homeless is seeking a Case Worker to work in our Elizabeth, NJ office. This position is a part-time position.
Duties and Responsibilities:
Assist clients with case management and case updates.
- Ensure that clients are receiving the highest level of care.
- Provide case management and case updates to clients.
- Maintain a high level of communication with clients.
- Home visits including in Hotels/Motels.
- Development of service plans for clients.
- Coordinate services for new clients.
- Assess clients for eligibility of agency programs/assistance
- Other duties as assigned.
Required Skills and Abilities:
-Must be bi-lingual in Spanish
·Ability to work cooperatively with all staff members including co-workers, supervisors, and other clients.
· Ability to handle multiple tasks at one time while remaining organized and paying attention to detail.
· Ability to read, write, speak, and understand English sufficiently to perform the job duties and satisfactorily answer questions from supervisors or other staff members when necessary.
· Ability to write legibly; complete job applications; complete intake forms; prepare client reports; collect client contact information; maintain accurate records; follow up on referrals; document activities; update client records as needed; respond effectively to client inquiries regarding services available; meet required standards for handling confidential information (i.e., HIPAA) regarding client matters (i.e., medical records, mental health services, etc.)
·Maintain a high level of communication with clients.
Job Type: Part-time
Pay: $45,000.00 - $50,000.00 per year
Expected hours: 16 – 20 per week
