Assist in the planning, development, implementation, staff supervision, and evaluation of the Houseless Outreach and Navigation service.


Your job also involves . . .

Assuring that program staff and community partners work effectively together toward defined goals and objectives of project requirements. Developing program objectives.

Provide outreach to individuals and families who are, or who are at risk for being, houseless, with the goal of connecting them to community resources so that they can achieve permanent and stable housing.

  • Work with other County departments and community programs in identifying individuals who need support, advocacy, and resource navigation to achieve permanent housing.
  • Provide support, advocacy, and resource navigation to identified individuals.
  • Once housed, provide or oversee on-going supports to individuals for as long as needed to assure housing stability.
  • Complete record keeping documentation using platforms determined by LC-HHS according to identified timelines.

Team Management and Development:

  • Asist in the recruitment, selection, training, and assignments program staff.
  • Supervise, support, and determine work assignments for program staff.
  • Direct work performance of assigned staff, conduct annual performance evaluations, and promote and facilitate workforce development.

Facilitate community mobilization through the development and sustaining of effective working relationships with partner agencies, community-based organizations, and individuals and groups that have an interest in serving houseless populations:

  • Sustain and expand partnerships with community-based organizations to ensure connections with groups and populations experiencing houselessness.
  • Plan, organize, and implement program changes.
  • Oversee data collection and analysis of information relevant to public and community health.

Other functions:

  • Participate in assigned HHS meetings.
  • Be the County's lead in the Annual Point in Time Homelessness Count.
  • Participate in the County's Emergency Preparedness planning, evaluation, response, and exercises.
  • Attend and participate in assigned community meetings to represent the issues, concerns, needs of the houseless community.

Minimum Education Level

Bachelors

Specifics: Bachelor's degree in a health-related field, or equivalent education in a related field


Minimum Experience Level

Two- Three years

Specifics: A minimum of 2 years of experience in a public health community-based health related program, serving marginalized and/or vulnerable populations.


Substitution Note: Any satisfactory combination of experience, training and education which clearly demonstrates the ability to perform the work, as determined by the Health and Human Services Director.


Driver's License

Oregon Driver's License required

Specifics: Ability to obtain within 30 days of hire.


Desirable Qualifications: Bilingual and/or bicultural competencies for the Latino population are preferred. Two years of supervisory or leadership experience preferred.


Knowledge of:

  • The issues, concerns, and needs of individuals experiencing houselessness.
  • Community resources and eligibility requirements for individuals needing resources.
  • Principles of engagement in serving individuals in the houselessness population
  • Basic understanding of trauma informed mental health and substance abuse assessment, engagement, and intervention
  • Community development, promotional, and organizational techniques.
  • Current trends, developments, and evidence-based practices in the public health fields of prevention and promotion.

Skill or ability to:

  • Demonstrate excellent organization, communication, and leadership skills as well as the ability to make decisions independently using good judgment, and excellent critical thinking skills.
  • Convey information, ideas, and facts both orally and in writing to supervisors, colleagues, and individuals, inside and outside the county, using language and a format the audience will best understand.
  • Utilize computers and other technology to communicate in written and verbal forms (e.g., word processing, texts, emails, spreadsheets, department-specific software).
  • Effectively relate well with others, including supervisors, colleagues, and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.
  • Take personal responsibility for the quality and timeliness of work. Show up to work on time, and follows instructions, policies, and procedures. Meet productivity standards, deadlines, and work schedules.

WORKING CONDITIONS

Job Conditions: This position requires attendance and participation in occasional evening and weekend meetings, conferences, and training events as well as overnight out of county meetings and events.


Work Location: Lincoln County


Travel: Regular travel throughout the county and occasionally out of County.
