What you bring:
- At least 2 years expereince in a behavioral health setting
- Background in nursing, social services, or patient referral management
- Strong sales experience
- Strong interpersonal skills
- Exceptional communication skills
- Ideally has longterm care and/or post acute care experience

As a Hospital Referral Navigator, you are instrumental to marketing and ensuring new residents have a smooth transition from acute to post-acute care. Your work will ensure our residents receive the high standard of care they have grown to expect at Legacy Healthcare by generating and managing referrals from hospitals, developing relationships with hospital discharge planning personnel, and developing and carrying out marketing and sales activities. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
