Job Description

Job Summary:

The Licensed Practical Nurse is responsible for the delivery of nursing care to achieve the goals as outlined in the participants Plan of Care and as ordered by the physician.

Duties/Responsibilities:

  • Provide nursing serves as indicated in Participants care plan or by physician/provider orders
  • Provide care as needed in the center or the participants’ home.
  • Identify areas of concern or needed improvement to maximize participant satisfaction and/or positive health outcomes, promotes those ideas to the Nurse Care Coordinator or IDT
  • Provides back-up to other nurses, home care and clinical staff as needed
  • Administers medication and provides medication education to participants and their families
  • Participates in family meetings, care plan meetings, and any committees identified by the Medical Director and / or PACE Program Director
  • Provides participants and families education as needed
  • Participates in discharge planning
  • Other duties as assigned.

  • Ability to adapt to changes necessitated by patient/staff situations
  • Ability to write legibly
  • Ability to interact in a positive manner and communicate well with patients, families, co-workers, supervisor, and other employees

Education, Licenses, Certifications and Experience:

  • Current LPN License
  • CPR Certification
  • ACLS Certification preferred
  • Graduate of an accredited LPN program
  • 1 year experience working with the frail and elderly population,

Safe Environment:

  • Demonstrate knowledge of Hospital specific and individual role in the fire/safety program including the following management plans:  safety, security, hazardous materials/waste, emergency preparedness, life safety, medical equipment and utility systems.
  • Participate in the Hospital policies and procedures relating to infection control for self, patients, and staff.
  • Observe proper body mechanics and safety precautions in all job duties.
  • Communicate information about incidents involving self, patients, staff and/or visitors to appropriate parties, using facility incident reporting systems as appropriate.
  • Demonstrates the safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation.

  • Attend and participate in all applicable training and development workshops including mandatory annual in-services/competencies.
  • Actively identify and participate in training, education and developmental activities to improve own knowledge and performance or the knowledge and performance of others.
