Job Description

Job Summary:

The Case Manager serves as the interdisciplinary team coordinator; facilitate the delivery of services to patients and their families; manage the financial resources of the patient by acting as a liaison to the payor; assume cost effectiveness of treatment provided by the team; facilitate and coordinate the discharge planning process; serve as the patient and family advocate; coordinate and direct effective communication regarding the patient’s care.

Duties/Responsibilities:

  • Assess for functional mobility for increased independence.
  • Involve in decision making and discharge plans with consideration for the psychosocial issues of life-changing events.
  • Provide instruction and education with regard to the age-related changes in hearing, vision, and cognition.
  • Assess mobility, strength, and ROM with regard to fall risks.
  • Assess skin integrity related to functional level and sensory impairments, and provides equipment as appropriate.
  • Assess vital signs and respiratory status, and allows for appropriate rest to optimize therapy.
  • Plan discharge with consideration for the psychosocial issues of life-changing events.
  • Any and all other duties  and responsibilities as assigned.  

 

Supervisory Responsibilities:

  • None.

 

Required Skills/Abilities:

  • Working knowledge of regulations and parameters of third-party reimbursement;
  • Exhibits tact, diplomacy, and team-player attitude working toward customer and hospital-wide objectives;
  • Protects confidentiality in all areas as applicable;
  • Ability to dictate patient documentation;
  • Ability to demonstrate skill in administration, accountability, decision-making, and time management;
  • Ability to interact in a positive manner and communicate well with patients, families, co-workers, supervisor, and other hospital employees;
  • Ability to coordinate the day-to-day function of the Department of Case Management..

 

Education, Licenses, Certifications and Experience:

  • Current RN or LSW Licensure preferred, or other disciplines as deemed appropriate.
  • CPR Certification.
  • Two to three year experience in medical rehabilitation, or acute care.

Safe Environment:

  • Demonstrate knowledge of Hospital specific and individual role in the fire/safety program including the following management plans:  safety, security, hazardous materials/waste, emergency preparedness, life safety, medical equipment and utility systems.
  • Participate in the Hospital policies and procedures relating to infection control for self, patients, and staff.
  • Observe proper body mechanics and safety precautions in all job duties.
  • Communicate information about incidents involving self, patients, staff and/or visitors to appropriate parties, using facility incident reporting systems as appropriate.
  • Demonstrates the safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation.
